Janmet 1000

Presentation: Each film-coated tablet contains sitagliptin phosphate monohydrate equivalent to 50 mg of sitagliptin and metformin hydrochloride 1000 mg. 

Indications:Janmet is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellituswhen treatment with both sitagliptin and metformin is appropriate.  

Dosage and Administration:   The dosage should be individualized based on the patient's current regimen, effectiveness, and tolerability, not exceeding the maximum recommendeddaily dose of sitagliptin (100 mg) and metformin (2000 mg). It is usually administered orally twice daily with meals to reduce the gastrointestinal side effects associated with metformin. The recommended starting dose in patients not currently treated with metformin isone tablet of Janmet 50/500 mg twice daily. In patients already treated with metformin, the starting dose of Janmet should provide the same daily dose of metformin, with the addition of sitagliptin 50 mg twice daily. For patients taking metformin 850 mg twice daily, the recommended starting dose is one tablet of Janmet 50/1000 mg twice daily.

Side Effects: Common side effects may include nausea, vomiting, diarrhea, abdominal pain, headache, and upper respiratory tract infection. Hypoglycemia may occur when Janmet is used in combination with insulin secretagogues (e.g., sulfonylureas) or insulin. Pancreatitis has been reported in some patients receiving sitagliptin. Lactic acidosis is a rare but serious potential side effect associated with metformin.

Precautions: Renal function should be assessed before initiating Janmet and periodically thereafter. Use is contraindicated in patients with severe renal impairment (eGFR < 30 mL/min). Caution is advised in patients with a history of pancreatitis. Hepatic impairment, heart failure, and excessive alcohol intake are also important considerations.

Storage: Store below 30°C, protected from moisture. Keep out of reach of children.
