Janmet XR 

Presentation: Janmet XR tablets contain a combination of Sitagliptin Phosphate Monohydrate (equivalent to Sitagliptin) and Metformin Hydrochloride Extended-Release. It is available in various strengths, such as 50mg/500mg, 50mg/1000mg, and 100mg/1000mg, presented in strips.

Indications: Janmet XR is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus when treatment with both sitagliptin and metformin is appropriate. It can be used:

• In patients not adequately controlled on metformin alone.
• In patients already treated with sitagliptin and metformin as separate tablets.

Dosage and Administration: Janmet XR should be taken orally once daily with a meal, preferably in the evening. The dosage should be individualized based on the patient's current regimen, effectiveness, and tolerability, not exceeding the maximum daily dose of 100 mg of sitagliptin and 2000 mg of metformin hydrochloride extended-release.

• For patients not currently treated with metformin: The recommended starting dose is 100 mg sitagliptin and 1000 mg metformin HCl extended-release once daily, with gradual dose escalation to reduce gastrointestinal side effects associated with metformin.
• For patients already treated with metformin: The starting dose should provide 100 mg sitagliptin and the previously prescribed dose of metformin.
• For patients taking metformin HCl immediate-release 850 mg twice daily or 1000 mg twice daily: The recommended starting dose is two 50 mg sitagliptin and 1000 mg metformin HCl extended-release tablets taken together once daily.
• Maintain the same total daily dose of sitagliptin and metformin when changing between immediate-release sitagliptin and metformin and Janmet XR.
• Swallow the tablets whole; do not crush, cut, or chew them.

Side Effects: Common side effects may include upper respiratory tract infection, stuffy or runny nose and sore throat, diarrhea, nausea, vomiting, and headache. Hypoglycemia can occur when Janmet XR is used in combination with insulin or sulfonylureas. Less common but potentially serious side effects include lactic acidosis, pancreatitis, severe joint pain, and allergic reactions.

Precautions: Janmet XR is contraindicated in patients with severe renal impairment (eGFR < 30 mL/min/1.73 m²), acute or chronic metabolic acidosis (including diabetic ketoacidosis), or known hypersensitivity to sitagliptin or metformin. It is not recommended for patients with type 1 diabetes or for the treatment of diabetic ketoacidosis. Use with caution in patients with moderate renal impairment (eGFR 30-45 mL/min/1.73 m²), a history of pancreatitis, heart failure, and excessive alcohol intake. Renal function should be assessed prior to initiation and periodically thereafter. Discontinue Janmet XR at the time of or prior to iodinated contrast imaging procedures in certain patients.

Storage: Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
