Janvia 50 

Presentation: Janvia 50 is presented as tablets, each containing 50mg of sitagliptin (as sitagliptin phosphate monohydrate). The tablets are usually film-coated and available in blister packs.The appearance (shape, color, markings) of the tablets may vary depending on the manufacturer in Bangladesh (e.g., ACME Laboratories Ltd.).

Indications:Janvia (sitagliptin) is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus.  It can be used:  

  • As monotherapyin patients inadequately controlled by diet and exercise alone and for whom metformin is inappropriate due to contraindications or intolerance. 
  • In combination with other glucose-lowering medications such as metformin, sulfonylureas, or thiazolidinediones when diet and exercise plus the single or dual therapy do not provide adequate glycemic control.

Dosage and Administration:The recommended dose of Janvia is usually 100 mg once daily.However, in patients with moderate renal insufficiency, the dose is 50 mg once daily, and in severe renal insufficiency or end-stage renal disease requiring hemodialysis or peritoneal dialysis, the dose is 25 mg once daily.

Janvia can be taken with or without food. It should be taken once daily at the same time each day to aid in remembering the dose. Follow your doctor's specific instructions regarding the dosage and timing of Janvia.

Side Effects: Common side effects reported with sitagliptin alone include headache, upper respiratory tract infection, and nasopharyngitis. When used in combination with other antidiabetic agents, the side effect profile may differ:

  • With sulfonylurea or insulin: Increased risk of hypoglycemia (low blood sugar).
  • Other side effects reported in combination therapies include nausea, vomiting, diarrhea, constipation, and peripheral edema.

Tell your doctor promptly if you experience any unusual or severe side effects, including severe abdominal pain which could indicate pancreatitis.

Precautions:Janvia (sitagliptin) is contraindicated in patients with a history of a serious hypersensitivity reaction to sitagliptin or any of the excipients.  

Caution should be exercised when Janvia is used in combination with sulfonylureas or insulin, as it may increase the risk of hypoglycemia.A dose reduction of the sulfonylurea or insulin may be considered.

Inform your doctor about all other medications you are taking, including herbal supplements, as there may be potential drug interactions.

The safety and efficacy of Janvia in children under 18 years have not been established.  

Use during pregnancy should only occur if clearly needed, and the potential benefits outweigh the risks to the fetus. It is not known if sitagliptin is excreted in human breast milk; caution should be exercised when administered to a nursing woman.  

Patients with renal impairment may require dose adjustments.  

Storage: Store Janvia tablets in a dry place, away from light and heat, below 30°C. Keep out of reach of children.
