Kofen (Ketotifen): An Anti-Allergic Agent for Asthma Prophylaxis and Allergic Conditions
Kofen contains Ketotifen, an anti-allergic agent with properties similar to sodium cromoglycate for the prophylactic treatment of asthma, and also possessing antihistamine properties. It is classified under Cromoglycate & related drugs.
How Kofen Works (Pharmacology)
Ketotifen exhibits a dual mechanism of action:
- Anti-anaphylactic/Mast Cell Stabilizing Properties: It has marked anti-anaphylactic properties and is effective in preventing asthmatic attacks. It is believed to exert an inhibitory effect on histamine release from mast cells, similar to mast cell stabilizers, thereby preventing the early phase allergic reactions.
- Antihistamine Properties: It exerts a sustained inhibitory effect on histamine reactions, acting as an H1-histamine receptor antagonist. This action is distinct from its anti-anaphylactic properties.
Key Pharmacological Features:
- Prophylactic Asthma Treatment: Clinical trials have shown that Ketotifen can reduce the number, severity, and duration of asthma attacks, with some patients even becoming completely free from attacks. It may also allow for a progressive reduction in corticosteroids and/or bronchodilators.
- Delayed Onset of Full Effect: The full prophylactic activity of Ketotifen may take several weeks to become established.
- Not a Rescue Medication: Ketotifen will not abort (stop) established attacks of asthma.
Key Indications & Benefits
Kofen is indicated for the following conditions:
- Prophylactic treatment of bronchial asthma.
- Symptomatic treatment of allergic conditions, including rhinitis (inflammation of the nasal lining) and conjunctivitis (inflammation of the conjunctiva, lining of the eye).
- Alleviating complications of itching, pain, and tenderness associated with neurofibroma.
- Symptomatic treatment of allergy such as hayfever and urticaria (hives).
Dosage & Administration
Always consult a registered physician for medication use.
- Adults: 1 mg twice daily with food. If necessary, the dose may be increased to 2 mg twice daily in severe cases.
- Children above 3 years: 1 mg twice daily with food. Patients known to be easily sedated should begin treatment with 0.5 to 1 mg at night for the first few days, or as directed by the physician.
- Use in elderly: Same as adult dose or as advised by the physician.

Contraindications:
- A reversible fall in platelet count has been observed in some patients receiving Ketotifen concomitantly with oral antidiabetic agents. Therefore, this combination should be avoided.
- Not recommended in pregnancy or when breastfeeding due to lack of evidence regarding teratogenic effects, despite no direct evidence of harm.
Side Effects:
- Common at beginning of treatment (usually disappear spontaneously after a few days): Drowsiness, and in isolated cases, dry mouth and slight dizziness.
Pregnancy & Lactation:
- Although there is no evidence of any teratogenic effect, Ketotifen is not recommended in pregnancy and lactation.
Precautions & Warnings:
- Continued Previous Treatment: It is important to continue previous asthma treatment for a minimum of two weeks after starting Kofen to avoid exacerbation of asthma. This is especially important for systemic corticosteroids and ACTH due to the possibility of adrenocortical insufficiency in steroid-dependent patients.
- Intercurrent Infection: If an intercurrent infection occurs, Kofen treatment must be supplemented by specific antimicrobial therapy.
- Impaired Reactions: During the first day of treatment, reactions may be impaired. Patients should be warned not to drive vehicles or operate machinery until the effect of Kofen on the individual is known.
- Alcohol: Patients should be advised to avoid alcoholic drinks.
- Potentiation of Effects: Kofen may potentiate the effects of sedatives, hypnotics, antihistamines, and alcohol.
Drug Interactions:
- Sedatives, Hypnotics, Antihistamines, and Alcohol: Kofen may potentiate their effects.
- Oral Antidiabetic Agents: A reversible fall in platelet count has been observed; therefore,concomitant use should be avoided.
Overdose Effects
- Reported features of overdosage: Confusion, drowsiness, headache, bradycardia (slow heart rate), respiratory depression.
- Management: Elimination of the drug with gastric lavage or emesis is recommended. Otherwise, general supportive treatment should be instituted.
Storage Conditions
Store in a cool and dry place, protect from light. Keep out of the reach of children.
