Lamogin 25 Tablet contains 25 mg of Lamotrigine.It is manufactured by Incepta Pharmaceuticals Ltd. in Bangladesh. 

Indications: Lamogin is an antiepileptic drug indicated for:

  • Epilepsy:
    • Adjunctive therapy in patients aged 2 years and older for partial-onset seizures, primary generalized tonic-clonic seizures, and generalized seizures of Lennox-Gastaut syndrome.  
    • Monotherapy for epilepsy in individuals aged 16 years and older.
  • Bipolar Disorder:In patients aged 18 years and older.  

Pharmacology:Lamotrigine's exact mechanism of action is not fully understood, but it is thought to work by inhibiting voltage-sensitive sodium channels, thereby stabilizing neuronal membranes and modulating the presynaptic release of excitatory neurotransmitters like glutamate and aspartate.  

Dosage and Administration: Dosage is highly individualized and should be determined by your doctor. Some general guidelines:

  • Epilepsy (Adults and Children over 12 years): Dosage escalation regimens vary depending on whether the patient is taking other antiepileptic drugs (like valproate or enzyme-inducing drugs such as carbamazepine, phenytoin, phenobarbital, or primidone). The usual maintenance dose ranges from 225-375 mg/day in one or two divided doses (for monotherapy) and varies for adjunctive therapy.
  • Epilepsy (Children 2-12 years): Dosage is based on weight and whether the child is taking other antiepileptic drugs.
  • Bipolar Disorder (Adults): Dosage escalation regimens are used to reach a maintenance dose, typically 200 mg once daily (for monotherapy) or 100 mg daily (for adjunctive therapy with valproate).

Lamogin tablets can be swallowed whole, chewed, or dispersed in water (for dispersible tablets, if available in this strength and formulation). Follow your doctor's instructions carefully. 

Contraindications:Lamogin is contraindicated in patients with hypersensitivity reactions (e.g., rash, angioedema) to lamotrigine or any of the inactive ingredients.  

Precautions:

  • Serious Skin Rashes:Lamotrigine can cause serious skin rashes, including Stevens-Johnson syndrome and toxic epidermal necrolysis, which can be life-threatening.The risk is higher in children and when lamotrigine is started at a high dose or the dose is increased too quickly, and when used with valproate.Discontinue lamotrigine at the first sign of a rash unless the rash is clearly not drug-related.  
  • Suicidal Behavior and Ideation: Antiepileptic drugs, including lamotrigine, increase the risk of suicidal thoughts or behavior. Monitor patients for any signs of depression or suicidal thoughts.
  • Aseptic Meningitis: Aseptic meningitis has been reported rarely.
  • Hemophagocytic Lymphohistiocytosis (HLH):HLH has been reported rarely.  
  • Blood Dyscrasias:Blood dyscrasias (e.g., neutropenia, thrombocytopenia) have been reported.Withdrawal Seizures:As with other antiepileptic drugs, abrupt discontinuation may precipitate seizures.  

Side Effects: Common side effects may include:

  • Dizziness  
  • Headache  
  • Diplopia (double vision)  
  • Ataxia (lack of coordination)  
  • Nausea  
  • Blurred vision  
  • Somnolence (drowsiness)  
  • Rash  

Drug Interactions:Lamotrigine can interact with several drugs:  

  • Valproate: Increases lamotrigine concentrations.
  • Carbamazepine, Phenytoin, Phenobarbital, Primidone, Rifampin: Decrease lamotrigine concentrations.
  • Estrogen-containing oral contraceptives: Decrease lamotrigine concentrations.
  • Lopinavir/ritonavir and Atazanavir/ritonavir: Decrease lamotrigine exposure.
  • Lamotrigine may affect the levels of other drugs, such as phenytoin and some oral contraceptives.  

Pregnancy and Lactation:Lamotrigine is Pregnancy Category C. It is excreted in human milk.A decision should be made whether to discontinue nursing or discontinue the drug, taking into account the importanceof the drug to the mother.  

Storage:Store below 25°C, in a dry place, protected from light. Keep out of reach of children.
