💊 Letrozole (Hormonal Chemotherapy)
Letrozole is a potent nonsteroidal aromatase inhibitor. In postmenopausal women, the body produces estrogen primarily by converting other hormones (androgens) into estrogen via the aromatase enzyme. Letrozole blocks this process, depriving estrogen-dependent breast cancer cells of the "fuel" they need to grow.
📋 Indications
- Early Breast Cancer: Used after surgery for postmenopausal women with hormone-receptor-positive cancer.
- Extended Therapy: For women who have already completed 5 years of Tamoxifen therapy.
- Advanced/Metastatic Cancer: First-line treatment for hormone-dependent breast cancer that has spread.
- Pre-operative Use: To shrink localized tumors before breast-conserving surgery.
🥄 Dosage & Administration
- Standard Dose:2.5 mg once daily.
- Duration: * For early breast cancer: Usually continued for 5 years or until the tumor returns.
- For metastatic disease: Continued as long as the treatment is effective.
- Timing: Can be taken with or without food.
- Hepatic/Renal Impairment: Generally no adjustment is needed for mild to moderate cases, but severe liver impairment (Child-Pugh C) requires close medical supervision.
⚠️ Contraindications
- 🛑 Pregnancy & Lactation: Strictly prohibited. It can cause fetal harm and malformations.
- 🛑 Pre-menopausal Status: Only indicated for women who have naturally or artificially reached menopause.
- 🛑 Severe Liver Dysfunction: Use is contraindicated in cases of severe hepatic failure.
⚠️ Side Effects
- Very Common: Hot flushes, increased cholesterol, joint and bone pain (arthralgia), and fatigue.
- Bone Health: Long-term use can lead to thinning of bones (osteoporosis), increasing the risk of fractures.
- General: Nausea, headache, dizziness, increased sweating, and weight gain.
- Rare/Serious: Blurred vision, anxiety, or yellowing of the skin/eyes (jaundice).

- Driving/Machinery: Because Letrozole can cause fatigue and dizziness, exercise caution when driving or operating heavy machinery until you know how the drug affects you.
