Lijenta-M 500 

Presentation:Lijenta-M 500 Tablet, available in Bangladesh, is a combination tablet containing Linagliptin 2.5 mg and Metformin Hydrochloride 500 mg.It is presented as a film-coated tablet for oral administration and is usually available in blister packs. The color and shape of the tablet may vary depending on the manufacturer.

Indications:Lijenta-M is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellituswhen treatment with both linagliptin and metforminis appropriate.  

Dosage and Administration:The dosage of Lijenta-M should be individualizedbased on the patient's current regimen, effectiveness, and tolerability. It is usually administered twice daily with meals to reduce the gastrointestinal side effects associated with metformin.

• For patients not currently treated with metformin: Initiate treatment with one tablet of Lijenta-M (2.5 mg linagliptin/500 mg metformin hydrochloride) twice daily.
• For patients already treated with metformin: Start with one tablet of Lijenta-M (2.5 mg linagliptin/500 mg metformin hydrochloride) twice daily, maintaining the same total daily dose of metformin.
• Patients already treated with linagliptin and metformin as separate tablets may be switched to Lijenta-M containing the same doses of each component.  
• The maximum recommended daily dose is linagliptin 5 mg and metformin 2000 mg.

Side Effects:Common side effects may include nasopharyngitis (common cold) and diarrhea.Hypoglycemia (low blood sugar) may occur when Lijenta-M is used in combination with other diabetes medications like insulin or sulfonylureas.⁷  

Precautions:Lijenta-M is contraindicated in patients with severe renal impairment (eGFR < 30 mL/min/1.73 m²), acute or chronic metabolic acidosis (including diabetic ketoacidosis), or a history of serious hypersensitivity reaction to linagliptin or metformin.It is not recommended for use in patients with type 1 diabetes mellitus. Caution should be exercised in patients with moderate renal impairment, heart failure, liver disease, and excessive alcohol intake due to the risk of lactic acidosis associated with metformin.
