Indications: Limpet (glimepiride) is an oral hypoglycemic agent belonging to the sulfonylurea class. It is indicated as an adjunct to diet and exercise to lower blood glucose in adults with type 2 diabetes mellitus whose hyperglycemia cannot be controlled by diet and exercise alone. Limpet may be used concomitantly with metformin when diet, exercise, and Limpet or metformin alone do not result in adequate glycemic control. Limpet is also indicated for use in combination with insulin to lower blood glucose in patients whose hyperglycemiacannot be controlled by diet and exercise in conjunction with an oralhypoglycemic agent.
Dosage and Administration: The dosage of Limpet 4 is governed by the desired blood sugar level. The dosage must be the lowest which is sufficient to achieve the desired metabolic control. The initialand maintenance doses are set based on the results of regularmonitoring of glucose in blood and urine. The usual starting dose is 1 mg once daily, taken immediately before a substantial breakfast or the first main meal. If necessary, the daily dose can be increased gradually based on regular blood sugar monitoring, usually at intervals of 1 to 2 weeks, and carried out stepwise (e.g., 1 mg -> 2 mg -> 3 mg -> 4 mg). The usual dose range in patients with well-controlled diabetes is 1 to 4 mg daily. A single daily dose is usually sufficient. It is very important not to skip meals after taking the drug.
Side Effects: The most common side effect is hypoglycemia (low blood sugar). Symptoms may include sweating, paleness, tremor, anxiety, unusual tiredness or weakness, confusion, difficulty concentrating, hunger, and visual disturbances. Other possible side effects include dizziness, asthenia (weakness), headache, and nausea.
Precautions: Limpet is contraindicated in patients with diabetic ketoacidosis, with or without coma, and in patients with known hypersensitivity to glimepiride or other sulfonylureas. It should be used with caution in patients with impaired renal or hepatic function, and in elderly or debilitated patients who may be more susceptible to hypoglycemia. Alcohol consumption should be avoided as it can potentiate the hypoglycemic effect. During periods of stress, such as fever, trauma, or surgery, a temporary change to insulin therapy may be necessary.
