Presentation:Each extended-release tablet contains Linagliptin INN 5 mg and Metformin Hydrochloride BP 1000 mg.Available in strips.
Indications:Indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellituswhen treatment with both linagliptin and metformin is appropriate.
Dosage and Administration:The dosageshould be individualized based on the patient's current regimen, glycemic control, and tolerability, not exceeding the maximum recommended daily dose of linagliptin 5 mg and metformin 2000 mg. The usual starting dose in patients not currently treated with metformin is one tablet once daily with a meal. In patients already on metformin, the starting dose is one tablet of Linamet ER 5/1000 once daily with a meal, providing the same total daily dose of metformin. Swallow whole with water; do not crush, cut, or chew. Dose escalation should be gradual to reduce gastrointestinal side effects associated with metformin.
Side Effects: Common side effects may include diarrhea, nausea, vomiting, and headache. Hypoglycemia may occur when used in combination with insulin or sulfonylureas. Other possible side effects include nasopharyngitis.
Precautions:Contraindicated in patients with severe renal impairment (eGFR < 30 mL/min/1.73 m²), acute metabolic acidosis, diabetic ketoacidosis, and known hypersensitivity to linagliptin or metformin.Not recommended to initiate treatment in patients with eGFR between 30-45 mL/min/1.73 m². Use with caution in patients with moderate renal impairment, history of pancreatitis, and heart failure. Temporary discontinuation may be required for surgical procedures or radiological examinations involving iodinated contrast media. Avoid excessive alcohol intake.
Storage: Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
