Lino-M XR 1000 

Presentation: Each extended-release tablet contains Linagliptin INN 2.5 mg and Metformin Hydrochloride BP 1000 mg. Available in strips.

Indications: Indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus when treatment with both linagliptin and metformin is appropriate.

Dosage and Administration: The dosage should be individualized based on the patient's current regimen, effectiveness, and tolerability, not exceeding the maximum recommended daily dose of linagliptin 5 mg and metformin hydrochloride 2000 mg. The usual starting dose in patients not currently treated with metformin is one tablet (linagliptin 2.5 mg / metformin hydrochloride 500 mg) twice daily or one tablet (linagliptin 2.5 mg / metformin hydrochloride 850 mg) twice daily. For Lino-M XR 1000, the usual dose is one tablet once daily with a meal. Patients using separate tablets of linagliptin 2.5 mg and metformin extended-release 1000 mg may switch to Lino-M XR 1000. Swallow the tablet whole; do not crush or chew. Gradual dose escalation is recommended to reduce gastrointestinal side effects associated with metformin. Assess renal function prior to initiation.

Side Effects: Common side effects may include diarrhea, nausea, vomiting, and headache. Hypoglycemia may occur when used in combination with insulin or sulfonylureas. Other possible side effects include nasopharyngitis.

Precautions: Contraindicated in patients with severe renal impairment (eGFR < 30 mL/min/1.73 m²), acute metabolic acidosis, diabetic ketoacidosis, and known hypersensitivity to linagliptin or metformin. Not recommended for use in patients with type 1 diabetes mellitus or for the treatment of diabetic ketoacidosis. Use with caution in patients with moderate renal impairment, history of pancreatitis, and heart failure. Avoid excessive alcohol intake.

Storage: Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
