Presentation: Tablets containing 667 mg of calcium acetate. Calcium acetate is a phosphate binder for oral administration. The tablets are typically white and may be film-coated. The appearance might vary based on the manufacturer (Square Pharmaceuticals PLC in Bangladesh).
Indications: Lofos 667 is indicated for the control of hyperphosphatemia in adult patients with end-stage renal disease (ESRD) who are undergoing dialysis. It works by binding with dietary phosphate in the gastrointestinal tract to form insoluble calcium phosphate, which is then excreted in the feces, thereby reducing serum phosphorus levels.
Dosage and Administration: The recommended initial dose for adult dialysis patients is 2 tablets with each meal. The dosage may be gradually increased by the physician to achieve the target serum phosphate level (typically below 6.0 mg/dL), provided that hypercalcemia does not develop. Most patients require 3 to 4 tablets with each meal. Lofos 667 tablets should be taken with food to maximize phosphate binding.
Side Effects: Patients may occasionally experience nausea during Lofos therapy. Hypercalcemia (high calcium levels in the blood) is a potential side effect and can range from mild (asymptomatic or with constipation, anorexia, nausea, vomiting) to severe (confusion, delirium, stupor, coma). Mild hypercalcemia is usually managed by reducing the dose or temporarily stopping Lofos. Isolated cases of pruritus (itching) have been reported, which may represent allergic reactions. The long-term effects on vascular or soft-tissue calcification have not been determined.
Precautions: Lofos 667 is contraindicated in patients with hypercalcemia. Serum calcium levels should be monitored, especially during dosage adjustment. If hypercalcemia develops, the dose should be reduced or treatment discontinued. Lofos should not be given to patients on digitalis, as hypercalcemia may precipitate cardiac arrhythmias. It may decrease the bioavailability of tetracycline antibiotics; therefore, these medications should not be taken at the same time. Use in pregnant women should only occur if clearly needed (Pregnancy Category C). Caution should be exercised during breastfeeding.
