Lopirel Plus (Clopidogrel & Aspirin): An Anti-platelet Combination
Lopirel Plus is a combination of two anti-platelet drugs: Clopidogrel and Aspirin. It is indicated to reduce the rate of myocardial infarction (MI) and stroke in patients with acute coronary syndrome (ACS), including unstable angina (UA), non-ST-elevation MI (NSTEMI), and ST-elevation MI (STEMI). It is also used in patients with a history of recent MI, recent stroke, or established peripheral arterial disease. Lopirel Plus belongs to the therapeutic class of anti-platelet drugs.
How Lopirel Plus Works (Pharmacology)
This combination works by inhibiting platelet activation and aggregation through two different mechanisms:
- Clopidogrel: This is a prodrug that becomes active in the liver. Its active metabolite irreversibly binds to the P2Y12 class of ADP receptors on platelets. This action prevents platelets from activating and clumping together, which is a key step in forming blood clots.
- Aspirin: This drug inhibits platelet aggregation by irreversibly inhibiting the enzyme cyclooxygenase. This action prevents the generation of thromboxane A2, a powerful inducer of platelet aggregation and vasoconstriction.
Key Indications & Benefits
Lopirel Plus is indicated for:
- Acute Coronary Syndrome (ACS): To reduce the rate of MI and stroke in patients with unstable angina, NSTEMI, and STEMI.
- Recent MI, recent Stroke, or established Peripheral Arterial Disease: To reduce the rate of MI and stroke.
Dosage & Administration
The dosage varies depending on the condition being treated. Always consult a registered physician for specific dosage instructions.
- For Acute Coronary Syndrome (UA/NSTEMI):
- The treatment should be initiated with a single 300 mg loading dose of Clopidogrel.
- This is followed by a daily maintenance dose of 75 mg of Clopidogrel.
- Aspirin (75 mg-325 mg once daily) should be initiated and continued in combination.
- For other indications:
- The recommended dose of Clopidogrel is 75 mg once daily, with or without food.
- No dosage adjustment is necessary for elderly or renal patients.

Contraindications:

- Lactation: It is unknown if Clopidogrel is excreted in human breast milk, but Aspirin is. The drug should be discontinued during breastfeeding.
Precautions & Warnings:
- Bleeding Risk: Lopirel Plus may prolong bleeding time.
- Reye's Syndrome: The Aspirin component may be a contributory factor in causing Reye's syndrome in children, especially those with chickenpox, influenza, or flu-like symptoms. It should not be given to children, especially those under 12, unless the benefits outweigh the risks.
Drug Interactions:
- NSAIDs: Concomitant use with nonsteroidal anti-inflammatory drugs (NSAIDs) may increase the risk of gastrointestinal blood loss.
- Warfarin: The safety of co-administration with warfarin has not been established, and caution should be exercised.
- Heparin: The co-administration of heparin does not necessitate a dose modification.
Overdose Effects
- Clopidogrel overdose: May lead to bleeding complications. Platelet transfusion may be used to restore clotting ability.
- Aspirin intoxication: Symptoms can range from dizziness and headache to severe metabolic acidosis. Treatment involves inducing vomiting, gastric lavage, or hemodialysis in severe cases.
