Indications

Loratadine tablet is a non-sedating antihistamine indicated for fast and effective relief of symptoms from:

  • Seasonal and perennial allergic rhinitis: Relieves symptoms like sneezing, nasal discharge, itching, and itchy/burning eyes.
  • Skin allergies: Including chronic urticaria (hives), and nettle rash in children over 2 years.

 

Pharmacology

Loratadine is a long-acting, non-sedative H1-histamine receptor antagonist. It selectively blocks peripheral H1-receptors, which are responsible for allergic reactions, without causing central nervous system effects like drowsiness. This allows for once-a-day administration.


 

Dosage & Administration

  • Adults and children over 12 years: One tablet once daily, preferably in the morning.
  • Children 2-12 years:
    • Body weight over 30 kg: One tablet once daily.
    • Body weight below 30 kg: Half a tablet once daily.
  • Children below 2 years: Not recommended, as safety and efficacy have not been established.

 

Interaction

  • Alcohol: Loratadine does not potentiate the effects of alcohol.
  • Other Drugs: Concomitant use with drugs that inhibit or are metabolized by certain liver enzymes (cytochromes P450 3A4 and 2D6), such as cimetidine, erythromycin, or ketoconazole, may increase Loratadine's plasma concentrations. However, no adverse effects have been reported.

 

Contraindications

Loratadine is contraindicated in patients with a known hypersensitivity or idiosyncrasy to its components.


 

Side Effects

The incidence of side effects, including sedation, is comparable to placebo.


 

Pregnancy & Lactation

There is no experience with its use in human pregnancy, so its use is not advisable. Loratadine is excreted in small amounts in breast milk, so nursing mothers are advised not to take the drug.


 

Precautions & Warnings

Caution should be exercised in patients with liver impairment or severe renal insufficiency (eGFR <30 ml/min).


 

Overdose Effects

Overdose (greater than 10 mg) in adults may lead to somnolence, tachycardia, and headache. In children, it may cause extrapyramidal signs and palpitations. In case of an overdose, symptomatic and supportive measures should be taken.
