Losucon M 1 

Presentation: Losucon M 1 Tablet, available in Bangladesh, is a combination medicine containing Glimepiride BP 1 mg and Metformin Hydrochloride BP 500 mg in a sustained-release formulation.

Indications: Losucon M is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellituswhentreatment with both glimepiride and metformin is appropriate.This may include patients whose diabetes is not adequately controlled on metformin alone or who are already treated with both glimepiride and metformin as separate tablets.

Dosage and Administration: The dosage of Losucon M must be individualized based on the patient's current treatment regimen, glycemic control, and tolerability, using the lowest effective dose. It is generally administered once daily with the morning meal.

  • Patients not currently treated with a combination of glimepiride and metformin: The usual starting dose is one tablet of Losucon M 1 once daily with breakfast.
  • Patients switching from separate tablets of glimepiride and metformin: The starting dose should be based on the patient's previous doses of glimepiride and metformin.
  • Dosage Titration: Dosage adjustments should be made gradually based on blood glucose response. It is recommended to increase the dose in increments of one tablet every 1-2 weeks.
  • Maximum Daily Dose: The maximum recommended daily dose is Glimepiride 8 mg and Metformin 2000 mg. As Losucon M 1 contains glimepiride 1 mg and metformin 500 mg, the maximum number of tablets per day would typically be determined by the maximum allowed dose of either component.
  • Administration: The tablet should be swallowed whole and should not be crushed, split, or chewed to maintain the sustained-release properties of metformin.

Side Effects: The most common side effect with glimepiride is hypoglycemia (low blood sugar). The risk is increased with missed meals, strenuous exercise, or concomitant use of other hypoglycemic agents or alcohol. Symptoms can include sweating, tremor, anxiety, confusion, and seizures. Gastrointestinal side effects are common with metformin, especially during initiation, and may include nausea, vomiting, diarrhea, abdominal pain, and loss of appetite. These are often transient. Less common side effects may include headache, dizziness, and allergic skin reactions. Rare but serious side effects associated with metformin include lactic acidosis, and long-term use may lead to vitamin B12 deficiency.

Precautions: Losucon M is contraindicated in patients with known hypersensitivity to glimepiride, metformin, other sulfonylureas, or sulfonamides, severe renal impairment (eGFR < 30 mL/min/1.73 m²), acute or chronic metabolic acidosis (including diabetic ketoacidosis), and hepatic impairment. It is not recommended for use in patients with type 1 diabetes mellitus or for the treatment of diabetic ketoacidosis. Caution should be exercised in patients with moderate renal impairment, heart failure, and in elderly, debilitated, or malnourished patients due to the increased risk of hypoglycemia and lactic acidosis. Alcohol consumption should be avoided. Certain medications can interact with Losucon M, affecting blood glucose control or increasing the risk of side effects. Regular monitoring of blood glucose levels and renal function is essential.

Storage: Store Losucon M 1 tablets in a cool (below 30°C) and dry place, protected from light and moisture. Keep this medication out of the reach of children. Do not use after the expiry date stated on the packaging.
