Losucon M 2 

Presentation: Losucon M 2 Tablet, available in Bangladesh, is a combination medicine containing Glimepiride 2 mg and Metformin Hydrochloride 500 mg.

Indications: Losucon M is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus when treatment with both glimepiride and metformin is appropriate. This includes patients whose type 2 diabetes is not adequately controlled on metformin alone or glimepiride alone, or who are already being treated with the combination of glimepiride and metformin as separate tablets.

Dosage and Administration: The dosage of Losucon M should be individualized based on the patient's current treatment regimen, glycemic control, and tolerability, using the lowest effective dose. It is generally administered once or twice daily with meals.

  • Patients not currently treated with metformin: The usual starting dose is one tablet of Losucon M 1/500 mg once daily with breakfast or the first main meal. Dosage may be adjusted based on blood glucose response.
  • Patients switching from metformin monotherapy: The starting dose of Losucon M should be based on the patient's previous dose of metformin. The addition of glimepiride 2 mg is usually initiated once daily.
  • Patients switching from sulfonylurea monotherapy (e.g., glimepiride): The starting dose of Losucon M should be based on the patient's previous dose of the sulfonylurea. The addition of metformin 500 mg is usually initiated once or twice daily.
  • Maximum Daily Dose: The maximum recommended daily dose of glimepiride is 8 mg and metformin is 2000 mg. With Losucon M 2/500, the maximum dose would typically be four tablets daily, usually divided into two doses. Always follow the physician's prescription.
  • Administration: The tablet should be swallowed whole and should not be crushed, split, or chewed. It is important to take it with meals to help prevent hypoglycemia and reduce gastrointestinal side effects.

Side Effects: Common side effects associated with metformin include gastrointestinal disturbances such as diarrhea, nausea, vomiting, and abdominal discomfort, particularly during initiation of therapy. The most common side effect associated with glimepiride is hypoglycemia (low blood sugar). Symptoms of hypoglycemia can include headache, dizziness, sweating, confusion, tremor, and hunger. Less common side effects may include taste disturbances, skin rash, and changes in liver function tests. Lactic acidosis is a rare but serious side effect associated with metformin.

Precautions: Losucon M is contraindicated in patients with severe renal impairment (eGFR < 30 mL/min/1.73 m²), acute or chronic metabolic acidosis (including diabetic ketoacidosis), a history of serious hypersensitivity reaction to glimepiride or metformin, and type 1 diabetes mellitus. It is also contraindicated in patients with conditions that can lead to lactic acidosis, such as severe heart failure, acute myocardial infarction, and severe infection. Caution should be exercised in patients with moderate renal impairment, liver disease, and excessive alcohol intake due to the increased risk of lactic acidosis associated with metformin and hypoglycemia associated with glimepiride. Regular monitoring of renal function is recommended.

Storage: Store Losucon M 2 tablets in a cool (below 30°C) and dry place, protected from light and moisture. Keep out of reach of children.
