Maacal-D

Presentation: Tablets containing a combination of calcium carbonate (derived from eggshell) and Vitamin D3 (cholecalciferol). It is available in different strengths depending on the formulation in Bangladesh, such as Calcium 500mg with Vitamin D3 200 IU, or Calcium 600mg with Vitamin D3 400 IU. The tablets are film-coated and may come in blister packs of 10 tablets per strip.

Indications: Indicated for the treatment and prevention of calcium and Vitamin D3 deficiency. It is used in conditions such as:

• Bone loss (osteoporosis) 
• Weak bones (osteomalacia, osteopenia) 
• Rickets 
• Decreased activity of the parathyroid gland (hypoparathyroidism) 
• Latent tetany (certain muscledisease)

It may also be used to ensure adequate calcium and Vitamin D3 intake in certain conditions such as:

• Pregnancy and lactation
• Post-menopausal osteoporosis
• Senile osteoporosis
• Drug-induced osteoporosis (e.g., due to Phenytoin, Phenobarbital, or Prednisolone-type drugs)

Dosage and Administration: The usual adult dose is one tablet in the morning and one tablet at night, or as directed by a physician. The dosage for children and adolescents should be directed by the physician. Maacal-D tablets are for oral administration and should be swallowed with water. It is generally recommended to take them with or after meals to enhance absorption.

Side Effects: Generally, orally administered Calcium Carbonate may be irritating to the gastrointestinal tract and may cause constipation. However, eggshell calcium, the source of calcium in Maacal-D, is reported to be safe for long-term use without causing significant gastrointestinal discomfort or constipation in most individuals. Rare side effects may include kidney stones, hypercalcemia (high calcium levels in the blood), alkalosis, and loss of appetite.

Precautions: Contraindicated in patients with hypercalcemia (high calcium levels in the blood), hyperparathyroidism (increased activity of the parathyroid gland), hypercalciuria (high levels of calcium in the urine), nephrolithiasis (kidney stones), hypersensitivity to any component of the preparation, severe renal insufficiency, and concomitant digoxin therapy (requires careful monitoring of serum calcium levels). Use with caution and as directed by a physician during pregnancy and lactation. Patients with a history of kidney stone formation should be recommended to increase their fluid intake.

Storage: Store in a cool (below 30°C) and dry place, away from light. Keep out of reach of children.
