Maxilax 

Presentation: Maxilax, available in Bangladesh, is a brand name primarily for products containing Lactitol Monohydrate. It is available as an oral powder and as an oral solution. The strength may vary depending on the formulation. For example, the oral solution may be available as 10 gm/15 ml. 

Indications: Maxilax (Lactitol Monohydrate) is indicated for the treatment of:

• Constipation, including chronic idiopathic constipation. It acts as an osmotic laxative, increasing water content in the large intestine, which softens the stool and promotes bowel movements.
• Hepatic encephalopathy, both acute and chronic portal systemic encephalopathy. It helps to reduce ammonia levels in the blood.

Dosage and Administration: The dosage and form of Maxilax depend on the condition being treated and the patient's age and response. Always follow your doctor's specific instructions.

• For Constipation (Oral Powder): The usual adult starting dose is 10-20 gm once daily with food. The dose may be adjusted to a maintenance dose of 5-10 gm once daily. For children, the dose should be as directed by the physician. The powder should be mixed with water or another suitable liquid before ingestion.

• For Constipation (Oral Solution): The usual adult dose is 15 ml twice daily initially, followed by a maintenance dose of 15-30 ml once daily. For children, the dose should be determined by the physician.

• For Hepatic Encephalopathy: The dosage is individualized and significantly higher, aiming to produce 2-3 soft stools per day. The oral powder is typically used for this indication, administered via a nasogastric tube if the patient is unable to swallow.

It is often recommended to take laxatives at bedtime as they may take some time to produce an effect.

Side Effects: Common side effects of Maxilax (Lactitol Monohydrate) may include flatulence, abdominal discomfort, nausea, and increased bowel frequency. Excessive doses may lead to diarrhea and potential electrolyte imbalance.

Precautions: Maxilax (Lactitol Monohydrate) should be used with caution in patients with:

• Galactosemia (a rare genetic disorder).
• Intestinal obstruction.
• Inflammatory bowel disease.
• Undiagnosed abdominal pain.
• Diabetes mellitus, as high doses can affect blood sugar levels.

Use in Pregnancy and Breastfeeding: Lactitol is generally considered safe for use during pregnancy and breastfeeding as it is poorly absorbed systemically. However, it is always best to consult a healthcare professional before using any medication during these periods.
