Merolin 5 Tablet contains Mirogabalin Besilate INN equivalent to Mirogabalin 5 mg. 
It is manufactured by Ziska Pharmaceuticals Ltd. in Bangladesh. 

Indications: Merolin is indicated for the treatment of:

• Peripheral neuropathic pain  
• Diabetic peripheral neuropathic pain  
• Postherpetic neuralgia  

Pharmacology: Mirogabalin selectively binds to the α2δ-1 subunit of voltage-gated calcium channels (VGCCs). This reduces calcium influx and neurotransmission in the dorsal root ganglia, inhibiting neurotransmitter release in presynaptic neuron endings, thus providing an analgesic effect.  

Dosage and Administration:

• The initial dose for adults is 5 mg orally twice daily.

If needed, the dose can be gradually increased by 5 mg at intervals of 1 week or longer.The dose may be adjusted according to the patient's age and symptoms in the range of 10 mg to 15 mg, administered twice daily. 

Renal Impairment:Dosage adjustments are needed in patients with renal dysfunction.

• Mild renal dysfunction: Initial dose 5 mg twice daily, increased by 5 mg at 1-week intervals to 10 mg twice daily.
• Moderate renal dysfunction: Initial dose 2.5 mg twice daily, increased by 2.5 mg at 1-week intervals to 7.5 mg twice daily.
• Severe renal dysfunction:Initial dose 2.5 mg once daily, increased by 2.5 mg at 1-week intervals to 7.5 mg once daily.  

Side Effects: Common side effects include:

• Somnolence (drowsiness)  
• Dizziness
• Headache

If any of the following serious symptoms occur, stop taking the medicine and see your doctor immediately:

• Lightheadedness, state close to sleep with impaired consciousness, loss of consciousness (dizziness, somnolence, unconsciousness).  
• General malaise, loss of appetite, nausea, vomiting, jaundice (liver dysfunction).  

Precautions:

• If an allergic reaction occurs, stop taking the medicine and consult with a doctor.
• Dose adjustment is needed in patients with renal dysfunction.  
• If taking any other medication, please consult with a doctor before administeringMirogabalin.  
• As this medicine may cause dizziness or somnolence, the patient should avoid operatingpotentially hazardous activities such as driving a car.  
• Elderly patients should be aware of falling and fracture.The patient should consult with the doctor ifsigns of blurred vision or double vision appear while taking this medication.  
• Use with caution in pregnant or potentially pregnant women, only if the therapeutic benefit outweighs the risks.Consider the therapeutic and breastfeeding benefits then consider continuing or discontinuing breastfeeding as it has been reported in animal studies that it is transferred into milk.

Drug Interactions:

• Co-administration of Merolin with Cimetidine or Probenecid may raise the Mirogabalin plasma concentration. Use with caution. 
• If Merolin is taken with Lorazepam, the depressive effects on the CNS may be potentiated. Avoid consuming alcohol while taking Merolin, as it may potentiate the impairment of motor skills and sedating effects of alcohol.

Contraindications:Mirogabalin is contraindicated in patientswith known hypersensitivity to Mirogabalin or any of its components.  

Storage: Do not store above 25°C. Protect from light. Keep out of the reach of children.
