Presentation:Met XR 750 is presented as extended-release tablets, each containing 750mg of metformin hydrochloride.The tablets are usually film-coated and available in blister packs.The appearance (shape, color, markings) of the tablets may vary depending on the manufacturer in Bangladesh (e.g., Opsonin Pharma Ltd.) The extended-release formulation is designed to release the medication slowly over time.
Indications:Met XR (metformin hydrochloride extended release) is indicated for the treatment of type 2 diabetes mellitus in adults, particularly in overweight patients, when diet and exercise alone have not been sufficient to control blood glucose levels.It can be used as monotherapy or in combination with other oral antidiabetic agents or insulin.
Dosage and Administration:The dosage of Met XR 750 is individualized based on blood glucose control and tolerability.The usual starting dose is 500mg once daily with the evening meal, which may be increased gradually in increments of 500mg weekly up to a maximum of 2000mg once daily with the evening meal, or 1000mg twice daily with meals. Patients already on immediate-release metformin may be switched to Met XR at the same total daily dose, up to a maximum of 2000mg once daily.
The tablets should be swallowed whole and should not be crushed, cut, or chewed. It is important to take Met XR with meals to help reduce gastrointestinal side effects. Follow your doctor's specific instructions regarding the dosage and timing of Met XR. Regular monitoring of blood glucose is essential.
Side Effects: Common side effects are gastrointestinal disturbances such as nausea, vomiting, diarrhea, abdominal pain, and loss of appetite, which are more frequent at the start of treatment and usually resolve spontaneously. A metallic taste may also occur. Less common but potentially serious side effects include lactic acidosis (symptoms include feeling very weak or tired, unusual muscle pain, trouble breathing, stomach pain with nausea and vomiting, feeling cold, and dizziness or lightheadedness) and vitamin B12 deficiency with long-term use.
Precautions: Met XR (metformin hydrochloride extended release) is contraindicated in patients with severe kidney disease (eGFR < 30 mL/min/1.73 m²), metabolic acidosis (including diabetic ketoacidosis), and known hypersensitivity to metformin. It is generally not recommended in patients with active liver disease or excessive alcohol intake.
Caution should be exercised in patients with moderate kidney impairment, heart failure, and in elderly patients. Temporary discontinuation may be necessary before surgery or certain medical procedures involving iodinated contrast agents.Avoid excessive alcohol consumption. Regular monitoring of kidney function is recommended.
Storage:Store Met XR 750 tablets in a dry place, away from light and heat, below 30°C.Keep out of reach of children.
