Metco IV 

Presentation:Metco IV is a brand name for Metronidazole intravenous infusion, available as a sterile solution containing Metronidazole 500 mg in 100 ml.It is usually supplied in glass bottles or plastic bags for intravenous administration. Metco IV is marketed by Eskayef Pharmaceuticals Ltd. in Bangladesh.

Indications:Metco IV (Metronidazole) is indicated for the treatment of serious infections caused by susceptible anaerobic bacteria and certain protozoa.This includes:

• Prevention of postoperative infections in contaminated or potentially contaminated surgery (e.g., colorectal, gynaecological).
• Treatment of septicaemia, bacteraemia, peritonitis, brain abscess, pelvic abscess, pelvic cellulitis, and postoperative wound infections caused by anaerobes.  
• Treatment of urogenital trichomoniasis and bacterial vaginosis.  
• Treatment of amoebiasis and giardiasis.  
• Other infections caused by susceptible anaerobic bacteria.

Dosage and Administration: Metco IV is administered intravenously by a healthcare professional. The usual dosage for adults and children over 12 years is 500 mg infused intravenously every 8 hours at a rate of 5 ml/minute.The maximum daily dose should not exceed 4 g. The duration of treatment is usually 7 days, but may be adjusted based on the type and severity of the infection. For surgical prophylaxis, a single dose is often given before surgery, followed by doses every 8 hours for up to 24 hours post-operatively.Dosage adjustments may be necessary for patients with renal or hepatic impairment and for children under 12 years based on body weight.

Side Effects: Common side effects may include nausea, vomiting, diarrhoea, abdominal discomfort, and a metallic taste. Less common side effects can include headache, dizziness, drowsiness, and skin rash. Rarely, more serious side effects such as peripheral neuropathy, seizures, and severe allergic reactions may occur.Patients may also experience a disulfiram-like reaction (abdominal cramps, nausea, vomiting, headache, flushing) if alcohol is consumed during or shortly after treatment.  

Precautions:Metco IV (Metronidazole) is contraindicated in patients with known hypersensitivity to metronidazole or other nitroimidazole derivatives.It should be used with caution in patients with active central nervous system disease, a history of blood dyscrasias, and severe hepatic impairment. Monitoring of serum levels may be necessary in elderly patients and those with altered pharmacokinetics.

Use in Pregnancy and Lactation: Metronidazole is classified as Pregnancy Category B. It should be used during pregnancy only if clearly needed. Metronidazole is excreted in human milk, and breastfeeding is generally not recommended during treatment and for a period after the last dose, depending on the guidelines.

Storage:Store below 30°C, protected from light.Do not freeze. Keep out of reach of children.
