Presentation: Extended-release tablets containing 500 mg metformin hydrochloride.
Indications: Management of type 2 diabetes mellitus in adults, especially in overweight patients, when diet and exercise alone are insufficient for adequate glycemic control. May be used as monotherapy or in combination with other oral antidiabetic agents or insulin.
Dosage and Administration: Usually initiated at 500 mg once daily with the evening meal. Dosage may be increased weekly in increments of 500 mg up to a maximum of 2000 mg once daily with the evening meal, or 1000 mg twice daily with meals. Swallow tablets whole; do not crush or chew. Individualized based on glycemic control and tolerability. Contraindicated in patients with eGFR below 30 mL/min/1.73 m², and not recommended to initiate in patients with eGFR between 30-45 mL/min/1.73 m². Assess renal function before starting and at least annually thereafter. Take with meals to reduce gastrointestinal side effects.
Side Effects: Common side effects include diarrhea, nausea, vomiting, flatulence, and abdominal discomfort, which may decrease over time. Metallic taste may occur. Long-term use may lead to vitamin B12 deficiency. Lactic acidosis is a rare but serious potential side effect.
Precautions: Contraindicated in severe renal impairment (eGFR < 30 mL/min/1.73 m²), metabolic acidosis (including diabetic ketoacidosis), and known hypersensitivity to metformin. Use with caution in patients with moderate renal impairment (eGFR 30-45 mL/min/1.73 m²), liver disease, heart failure, and excessive alcohol intake due to increased risk of lactic acidosis. Temporary discontinuation may be required for surgical procedures or radiologic studies involving iodinated contrast agents.
Storage: Store below 30°C, in a dry place, protected from light and moisture. Keep out of reach of children.
