Methipred 16 

Presentation: Methipred 16 Tablet, available in Bangladesh, contains Methylprednisolone 16 mg.

Indications: Methipred is a corticosteroid indicated for the treatment of a wide range of conditions due to its anti-inflammatory and immunosuppressant effects. These include:

  • Endocrine Disorders:Primary or secondary adrenocortical insufficiency, congenital adrenal hyperplasia, nonsuppurative thyroiditis, hypercalcemia associated with cancer. 
  • Rheumatic Disorders:As adjunctive therapy for short-term administrationin acute episodes or exacerbations of rheumatoid arthritis, psoriatic arthritis, ankylosing spondylitis, acute gouty arthritis, and others.
  • Collagen Diseases: During an exacerbation or as maintenance therapy in systemic lupus erythematosus, systemic dermatomyositis, and acute rheumatic carditis. 
  • Dermatologic Diseases:Pemphigus, bullous dermatitis herpetiformis, severe erythema multiforme,exfoliative dermatitis, severe psoriasis, and others.
  • Allergic States: Control of severe or incapacitating allergic conditions such as seasonal or perennial allergic rhinitis, bronchial asthma, contact dermatitis, atopic dermatitis,and drug hypersensitivity reactions. 
  • Ophthalmic Diseases:Severe acute and chronic allergic and inflammatory processes involving theeye.
  • Respiratory Diseases: Symptomatic sarcoidosis, berylliosis, aspiration pneumonitis. 
  • Hematologic Disorders:Idiopathic thrombocytopenic purpura in adults, secondary thrombocytopenia in adults, acquired(autoimmune) hemolyticanemia, and others.
  • Neoplastic Diseases: For palliative management of certain leukemias and lymphomas.
  • Edematous States: To induce diuresis or remission of proteinuria in nephrotic syndrome.
  • Gastrointestinal Diseases: To tide the patient over a critical period of ulcerative colitis and regional enteritis.
  • Nervous System: Acute exacerbations of multiple sclerosis.

Dosage and Administration: The initial dosage of Methipred 16 may vary depending on the specific disease being treated. Dosage requirements are variable and must be individualized based on the disease and the patient's response.

  • The usual oral starting dose for adults may range from 4 to 48 mg per day. In some cases, higher doses may be needed. For multiple sclerosis exacerbations, a common regimen is 160 mg daily for one week, followed by 64 mg every other day for 1 month.
  • The initial dose should be maintained or adjusted until a satisfactory response is noted.
  • After a favorable response is achieved, the maintenance dose should be determined by gradually reducing the initial dose to the lowest dose that will maintain an adequate clinical response.
  • Methipred tablets should be taken orally with a glass of water. It is often recommended to take it with food to minimize stomach upset.
  • Do not stop taking Methipred suddenly, especially if you have been on it for a prolonged period, as this can lead to withdrawal symptoms. The dose should be tapered gradually under the supervision of a physician.

Side Effects: Common side effects may include increased appetite, weight gain, fluid retention, mood changes, nervousness, restlessness, insomnia, acne, and increased sweating.

Less frequent or more serious side effects can include:

  • High blood pressure
  • Increased blood sugar levels
  • Weakening of bones (osteoporosis)
  • Increased risk of infections
  • Cataracts and glaucoma (with long-term use)
  • Muscle weakness
  • Thinning of the skin, easy bruising
  • Poor wound healing
  • Adrenal suppression

Precautions: Methipred is contraindicated in patients with systemic fungal infections and known hypersensitivity to methylprednisolone or any of its components.

Caution should be exercised in patients with:

  • Diabetes mellitus
  • Hypertension
  • Heart failure
  • Renal insufficiency
  • Liver disease
  • Peptic ulcer disease or a history of it
  • Osteoporosis
  • Glaucoma
  • Cataracts
  • Active or latent tuberculosis
  • Untreated systemic infections
  • Herpes simplex infection of the eye
  • Psychiatric disturbances

Use in pregnancy and breastfeeding should be carefully considered, and only if the potential benefit outweighs the risk to the fetus or infant. Children on long-term therapy should be closely monitored for growth retardation.

Storage: Store Methipred 16 tablets in a cool (below 30°C) and dry place, protected from light and moisture. Keep out of reach of children.
