Indications
Mig is indicated for the treatment of various types of neuropathic pain, including:
Diabetic Peripheral Neuropathic Pain (DPNP)
Postherpetic Neuralgia (PHN)
Peripheral Neuropathic Pain (PNP)
Pharmacology
Mirogabalin is a gamma amino acid derivative that acts by selectively binding to the α2δ subunits of voltage-gated calcium channels (VGCCs). This mechanism reduces calcium influx and inhibits neurotransmitter release, thereby decreasing the hyperexcitability of central nervous system (CNS) neurons. It is rapidly absorbed and primarily excreted unchanged by the kidneys.
Dosage & Administration (Adults)
The usual initial dose is 5 mg twice daily. The dose should be gradually increased by 5 mg at intervals of at least one week, up to a maintenance dose of 10 mg to 15 mg twice daily. Dosing must be adjusted for patients with renal impairment based on the severity of their condition.
Contraindications & Warnings
Contraindicated in patients with hypersensitivity to Mirogabalin.
The drug may cause side effects such as somnolence (drowsiness), dizziness, edema, and weight gain.
Patients should avoid driving or operating dangerous machinery due to the risk of dizziness and somnolence. Elderly patients should be particularly cautious due to the risk of falls.
Alcohol should be avoided as it may potentiate the CNS depressant effects of Mig.
Use in pregnant or potentially pregnant women is advised only if the therapeutic benefit outweighs the risk.
Side Effects
The most reported side effects include somnolence, dizziness, edema, and weight gain. Serious adverse reactions, though rare, include unconsciousness, liver dysfunction (jaundice), and blurred/double vision.
