Presentation: Misotol is a brand name primarily known for containing Misoprostol. Each tablet typically contains Misoprostol BP 200 mcg. It is usually available in blister packs. The marketing company in Bangladesh may vary.

Indications: Misotol (Misoprostol) has several indications, including:

• Prevention of NSAID-induced gastric ulcers: In patients at high risk of complications from gastric ulcers.
• Healing of gastric and duodenal ulcers.
• Medical abortion: Used to terminate pregnancy up to 63 days of gestation, often in combination with Mifepristone, under strict medical supervision.
• Management of incomplete abortion.
• Induction of labor.
• Treatment of postpartum hemorrhage.
• Cervical dilation before aspiration or curettage.

Dosage and Administration: Dosage varies significantly depending on the indication and route of administration (oral, vaginal, sublingual, rectal). Always follow the instructions provided by a physician.

• For prevention of NSAID-induced ulcers: The usual adult oral dose is 200 mcg four times daily with food.
• For healing ulcers: 200 mcg orally four times a day with food.
• For medical abortion: Usually 800 mcg vaginally, sublingually, or orally, following Mifepristone, under strict medical supervision.
• For incomplete abortion: 400-600 mcg orally or vaginally as a single dose or repeated doses as directed.

Side Effects: Common side effects often involve the gastrointestinal system and may include diarrhea, abdominal pain, nausea, vomiting, and flatulence. Gynaecological side effects such as vaginal bleeding, cramping, and menstrual disorders may also occur. Other side effects may include headache, dizziness, fever, and chills.

Precautions: Misotol (Misoprostol) is contraindicated in women who are pregnant when used for its anti-ulcer properties due to its abortifacient properties. It is also contraindicated in patients with a history of allergy to prostaglandins. Caution should be used in patients with pre-existing cardiovascular disease. It should be used with caution in patients with inflammatory bowel disease.

Use in Pregnancy and Lactation: Misotol is contraindicated during pregnancy when used for ulcer prevention or treatment. It is used to terminate pregnancy under medical supervision. Misoprostol can cause uterine contractions, miscarriage, premature birth, or birth defects if taken during pregnancy. It is excreted in breast milk; however, no contraindication has been established. Some experts recommend caution, as it may cause diarrhea in the breastfed infant.
