  • Indications: Mitadip Tablets are indicated for the treatment of major depressive disorder (MDD).

  • Pharmacology:

    • Pharmacodynamics: The mechanism of action of Mirtazapine, like other drugs effective in treating major depressive disorder, is unknown. Preclinical studies suggest that Mirtazapine enhances central noradrenergic and serotonergic activity. Mirtazapine acts as an antagonist at central presynaptic α2-adrenergic inhibitory autoreceptors and heteroreceptors, increasing central noradrenergic and serotonergic activity. It is also a potent antagonist of 5-HT2 and 5-HT3 receptors and a potent antagonist of histamine (H1) receptors, which may explain its sedative effects.

    • Pharmacokinetics: Mirtazapine is rapidly and well-absorbed after oral administration, reaching peak plasma levels in about 2 hours. It is approximately 85% bound to plasma proteins. The mean elimination half-life is 20-40 hours. Mirtazapine is extensively metabolized and eliminated via urine and feces.

  • Dosage & Administration:

    • Adult dose: The recommended starting dose is 15 mg/day, administered as a single dose, preferably in the evening or prior to sleep. The effective dose range is generally 15 to 45 mg/day. Dose changes should be made at intervals of not less than 1 to 2 weeks.

    • Use in children: Mirtazapine is not recommended for use in children.

    • Missed Dose: If a dose is missed, take it as soon as remembered, unless it is almost time for the next dose. Do not take a double dose.

  • Interaction:

    • Mitadip has clinically significant drug-drug interactions with Monoamine Oxidase Inhibitors (MAOI) and other serotonergic drugs.

    • Mitadip may interrupt the metabolism or activity of Carbamazepine, Phenytoin, or Cimetidine.

    • Patients should avoid alcohol and diazepam while taking Mitadip.

  • Contraindications:

    • Hypersensitivity: Mirtazapine is contraindicated in patients with known hypersensitivity to mirtazapine or any of the excipients.

    • Monoamine Oxidase Inhibitors: The concomitant use of Mirtazapine and a monoamine oxidase (MAO) inhibitor is contraindicated. Mirtazapine should not be used within 14 days of initiating or discontinuing therapy with an MAOI.

  • Side Effects: The most common side effects of Mitadip are dizziness, drowsiness, dry mouth, increased appetite, and weight gain.
