Mitaprex 7.5 Tablet contains Mirtazapine 7.5 mg. It is manufactured by Incepta Pharmaceuticals Ltd. in Bangladesh.

Indications:

Mitaprex (Mirtazapine) is indicated for the treatment of major depressive disorder (MDD).

Pharmacology:

Mirtazapine is a tetracyclic antidepressant that enhances central noradrenergic and serotonergic activity. It acts as an antagonist at central presynaptic α₂-adrenergic inhibitory autoreceptors and heteroreceptors, which is thought to result in an increase in norepinephrine and serotonin release. Mirtazapine is also a potent antagonist of 5-HT₂ and 5-HT₃ serotonin receptors and histamine (H₁) receptors. It has a moderate antagonistic effect on peripheral α₁-adrenergic and muscarinic receptors.

Dosage and Administration:

The recommended starting dose for adults is 15 mg daily, administered as a single dose, preferably in the evening or prior to sleep. The dose may be increased gradually depending on clinical response, usually at intervals of 1-2 weeks. The effective dose range is generally 15 to 45 mg per day.

Side Effects:

The most commonly reported adverse effects with Mirtazapine include:

  • Somnolence (drowsiness)
  • Increased appetite
  • Weight gain
  • Dry mouth
  • Dizziness

Other possible side effects may include constipation, nausea, increased cholesterol levels, and abnormal dreams.

Precautions:

  • Use with caution in patients with a history of seizures, mania/hypomania, hepatic or renal impairment, cardiac disorders, hypotension, diabetes mellitus, or a history of bipolar disorder.
  • Mirtazapine may impair the ability to drive or operate machinery due to drowsiness.
  • Avoid alcohol consumption as it can enhance the sedative effects of mirtazapine.
  • Monitor for signs of suicidal ideation or behavior, especially during the initial months of treatment and during dose changes.

Contraindications:

Mitaprex is contraindicated in patients with:

  • Known hypersensitivity to mirtazapine or any of the excipients.
  • Concomitant use with monoamine oxidase inhibitors (MAOIs) or within 14 days of discontinuing an MAOI.

Drug Interactions:

Mirtazapine can interact with various medications, including:

  • MAOIs: Increased risk of serotonin syndrome.
  • Other serotonergic drugs: Increased risk of serotonin syndrome.
  • CYP3A4 inhibitors (e.g., ketoconazole, erythromycin): May increase mirtazapine plasma levels.
  • CYP3A4 inducers (e.g., carbamazepine, phenytoin): May decrease mirtazapine plasma levels.
  • Alcohol and benzodiazepines: May enhance the sedative effects of mirtazapine.

Pregnancy and Lactation:

Mirtazapine should be used during pregnancy only if the potential benefit outweighs the risk to the fetus. It is generally not recommended during breastfeeding as it may pass into breast milk.

Storage:

Store at 15°C-30°C (59°F-86°F), protected from light and moisture. Keep out of reach of children.
