Modesto XR 3 Tablet contains 3 mg of Paliperidone in an extended-release formulation. It is manufactured by Incepta Pharmaceuticals Ltd. in Bangladesh. 

Indications: Modesto XR is indicated for the treatment of: 

• Schizophrenia, including acute treatment and recurrence prevention. 
• Treatment of schizoaffective disorder as monotherapy and in combination with antidepressants or mood stabilizers. 

Dosage and Administration:

• Adults: The initial dose for schizophrenia and schizoaffective disorder is usually 6 mg once daily. The recommended dose ranges from 3 to 12 mg once daily, with a maximum dose of 12 mg/day. 
• Adolescents (weighing less than 51 kg): The initial dose for schizophrenia is 3 mg once daily. The recommended dose ranges from 3 to 6 mg once daily, with a maximum dose of 6 mg/day. 
• Adolescents (weighing 51 kg or more): The initial dose for schizophrenia is 3 mg once daily. The recommended dose ranges from 3 to 12 mg once daily, with a maximum dose of 12 mg/day. 

Modesto XR extended-release tablets can be taken with or without food. They must be swallowed whole with liquids and should not be chewed, divided, or crushed. 

Side Effects: The most common adverse reactions associated with Paliperidone include: 

• Tachycardia (fast heart rate) 
• Akathisia (restlessness and an urge to move) 
• Somnolence (drowsiness) 
• Tremor 
• Dystonia (muscle contractions) 
• Cogwheel rigidity (stiffness with jerky movements) 
• Anxiety 
• Increased weight 
• Dyspepsia (indigestion) 
• Constipation 
• Nasopharyngitis (inflammation of the nose and throat) 

Precautions: Caution should be exercised when Modesto XR is prescribed in patients with: 

• Cardiovascular diseases 
• QT prolongation 
• Neuroleptic malignant syndrome 
• Tardive dyskinesia 
• Hyperglycemia 
• Dyslipidemia 
• Obesity 
• Hyperprolactinemia 
• Leukopenia, neutropenia, agranulocytosis 
• Seizures 
• Potential for cognitive and motor impairment 

Modesto XR should be used with caution in combination with other centrally acting drugs and alcohol. It may antagonize the effect of Levodopa and other dopamine agonists. 

Contraindications: Modesto XR is contraindicated in patients with a known hypersensitivity to paliperidone or risperidone, or to any of the excipients in the formulation. 

Pregnancy and Lactation: Paliperidone is classified as Pregnancy Category C. It should be used during pregnancy only if the potential benefits justify the potential risk to the fetus. Paliperidone is excreted in human breast milk, and breastfeeding is generally not recommended during treatment. 

Drug Interactions: Modesto XR may interact with various medications. It's important to inform your doctor about all the medicines you are currently taking. Significant interactions can occur with: 

• Strong CYP3A4/P-glycoprotein (P-gp) inducers (e.g., carbamazepine) 
• Divalproex sodium 
• Other centrally acting drugs and alcohol 
• Levodopa and other dopamine agonists 
• Drugs that have the potential to induce orthostatic hypotension 
• Drugs known to lower the seizure threshold 
• Drugs that prolong the QT interval 

Storage: Store in a cool and dry place, away from light. Keep out of reach of children.
