Modesto XR 6 Tablet contains Paliperidone 6 mgin an extended-release formulation. 
It is manufactured by Incepta Pharmaceuticals Ltd.  

Indications: Modesto XR (Paliperidone) is indicated for:

  • Schizophrenia, including acute treatment and recurrence prevention in adults and adolescents (12 years and older).  
  • Schizoaffective disorderas monotherapy and in combination with antidepressants or mood stabilizers in adults.  

Pharmacology: Paliperidone is a psychotropic agent belonging to the benzisoxazole derivatives chemical class. It is the major active metabolite of Risperidone.Paliperidone acts as a centrally active dopamine Type 2 (D₂) antagonist and has predominant serotonin Type 2 (5-HT₂A) antagonist activity.It also acts as an antagonist at α₁ and α₂ adrenergic receptors and H₁ histaminergic receptors.  

Dosage and Administration:

  • Adults: The initial dose for both schizophrenia and schizoaffective disorder is usually 6 mg once dailyin the morning.The recommended dose range is 3-12 mg once daily, with a maximum dose of 12 mg/day. Dosage adjustments are made based on individual response and tolerability.   
  • Adolescents (12 years and older) for Schizophrenia:
    • Weight < 51 kg: Initial dose is 3 mg once daily, with a recommended range of 3-6 mg/day and a maximum of 6 mg/day.
    • Weight ≥ 51 kg: Initial dose is 3 mg once daily, with a recommended range of 3-12 mg/day and a maximum of 12 mg/day.
  • Modesto XR tablets should be swallowed whole with liquid and can be taken with or without food. They should not be chewed, divided, or crushed.

Side Effects: The most common adverse reactions associated with Modesto XR (Paliperidone) include:

  • Tachycardia (fast heart rate)
  • Akathisia (restlessness and an urge to move)
  • Somnolence (drowsiness)
  • Tremor
  • Dystonia (muscle spasms)
  • Cogwheel rigidity (stiffness with jerky movements)
  • Anxiety
  • Weight increase
  • Dyspepsia (indigestion)
  • Constipation
  • Nasopharyngitis (common cold)  

Precautions: Caution should be exercised when Modesto XR is prescribed in patients with:

  • Cardiovascular diseases
  • QT prolongation  
  • Neuroleptic malignant syndrome (NMS)  
  • Tardive dyskinesia (involuntary movements)
  • Hyperglycemia (high blood sugar)  
  • Dyslipidemia (abnormal lipid levels)
  • Obesity  
  • Hyperprolactinemia (high prolactin levels)
  • Leukopenia, Neutropenia, Agranulocytosis (low white blood cell counts)  
  • Seizures
  • Potential for Cognitive & Motor Impairment

Drug Interactions:

  • Modesto XR should be used with caution with other centrally acting drugs and alcohol.  
  • It may antagonize the effect of Levodopa and other dopamine agonists.
  • Additive effects on orthostatic hypotension may occur with other drugs that lower blood pressure.
  • Strong CYP3A4/P-glycoprotein (P-gp) inducers (e.g., carbamazepine) may decrease paliperidone levels, requiring a dose increase of Modesto XR.Conversely, discontinuing the inducer may require a dose decrease.  
  • Co-administration with divalproex sodium can increase paliperidone levels. Dose adjustment of Modesto XR may be necessary.   

Pregnancy and Lactation:

  • Pregnancy Category C: Paliperidone should be used during pregnancy only if the potential benefits justify the potential risk to the fetus.
  • Paliperidone is excreted in human breast milk. The benefits of breastfeeding should be weighed against the potential risks to the infant.   

Storage: Store in a cool and dry place, away from light. Keep out of reach of children.
