Monas OFT (Montelukast): A Leukotriene Receptor Antagonist

Monas OFT contains Montelukast, a selective and orally active leukotriene receptor antagonist. It is indicated for the prophylaxis and chronic treatment of asthma, the acute prevention of exercise-induced bronchoconstriction (EIB), and the relief of symptoms of seasonal and perennial allergic rhinitis. Monas OFT belongs to the therapeutic class of leukotriene receptor antagonists.


How Monas OFT Works (Pharmacology)

Montelukast's mechanism of action is to block the effects of leukotrienes:

  • Inhibits Leukotriene Receptors: It selectively inhibits the cysteinyl leukotriene receptor (CysLT1).
  • Blocks Inflammatory Mediators: Cysteinyl leukotrienes (LTC4, LTD4, LTE4) are inflammatory substances released by various cells in the body, which play a major role in the pathophysiology of asthma and allergic rhinitis.
  • Reduces Symptoms: By blocking the effects of these leukotrienes, Montelukast reduces airway edema, smooth muscle contraction, and inflammation, thereby relieving the signs and symptoms of asthma and allergic rhinitis.

Dosage & Administration

  • Adults and adolescents (15 years and older): 10 mg tablet once daily.
  • Pediatric patients (6 to 14 years): 5 mg tablet once daily.
  • Pediatric patients (2 to 5 years): 4 mg tablet once daily.
  • Pediatric patients (6 months to 5 years): 4 mg oral granules once daily.

Contraindications:

  • Hypersensitivity to any component of the product.

Side Effects:

  • Common: Diarrhea, fever, headache, nausea, vomiting, skin reactions, and upper respiratory tract infections.
  • Uncommon: Anxiety, depression, dizziness, dry mouth, sleep disorders, and irritability.
  • Rare: Angioedema, hallucinations, suicidal tendencies, and liver disorders.

Pregnancy & Lactation:

  • Pregnancy: It should be used during pregnancy only if clearly needed, as there are no adequate and well-controlled studies in pregnant women.
  • Lactation: Caution should be exercised when given to a nursing mother, as it is unknown if it is excreted in human milk.

Precautions & Warnings:

  • Acute Asthma Attacks: Monas OFT is not indicated for the reversal of bronchospasm in acute asthma attacks. Patients should have appropriate rescue medication available.
  • Corticosteroids: It should not be abruptly substituted for inhaled or oral corticosteroids. The dose of inhaled corticosteroids should be reduced gradually under medical supervision.
  • Aspirin Sensitivity: Patients with aspirin sensitivity should continue to avoid aspirin or NSAIDs while taking Monas OFT.
  • Psychiatric Effects: Rare reports of neuropsychiatric events, including suicidal tendencies, exist.

Drug Interactions:

  • Monas OFT has been used with many common asthma treatments without adverse reactions.
  • CYP450 Inducers: Drugs that induce hepatic metabolism, such as phenobarbital or rifampin, can decrease the effectiveness of Monas OFT. Clinical monitoring is advised.

Overdose Effects

  • Overdose reports have been largely free of severe adverse effects, with the most common symptoms being consistent with the drug's safety profile (e.g., abdominal pain, headache).
  • In the event of an overdose, supportive measures should be employed.

Storage Conditions

Store in a cool and dry place below 30°C, protected from light and moisture. Keep out of the reach of children.
