Napa Syrup (120 mg/5 ml) contains Paracetamol (Acetaminophen) 120 mg per 5 ml. It is manufactured by Beximco Pharmaceuticals Ltd. in Bangladesh. 

Indications:

Napa Syrup is a widely used analgesic (pain reliever) and antipyretic (fever reducer). It is indicated for: 

  • Fever: Including fever associated with the common cold, influenza, and post-vaccination fever. 
  • Mild to moderate pain: Such as headache, toothache, earache, body ache, muscle pain (myalgia), nerve pain (neuralgia), pain during menstruation (dysmenorrhoea), sprains, colic pain, back pain, and post-operative pain. 
  • It is also indicated for pain and stiffness associated with rheumatic and osteoarthritic conditions. 

Pharmacology:

Paracetamol primarily acts in the Central Nervous System (CNS). It increases the pain threshold by inhibiting the synthesis of prostaglandins, which are chemicals involved in pain signal transmission. Its antipyretic effect is due to its action on the hypothalamus, the brain's heat-regulating center, leading to peripheral vasodilation and sweating, which helps to lower body temperature. Unlike NSAIDs, it has weak anti-inflammatory activity in peripheral tissues. 

Dosage and Administration:

Napa Syrup is taken orally. It can be administered with or without food. The dosage varies with age and weight. Do not exceed 5 doses daily, and do not use for more than 3 days for fever or 5 days for pain without consulting a doctor. 

  • Children under 3 months: 10 mg/kg body weight (reduce to 5 mg/kg if jaundiced), 3 to 4 times daily. 
  • 3 months to below 1 year: ÂŊ to 1 teaspoonful (60 mg - 120 mg) 3 to 4 times daily. 
  • 1-5 years: 1-2 teaspoonfuls (120 mg - 240 mg) 3 to 4 times daily. 
  • 6-12 years: 2-4 teaspoonfuls (240 mg - 480 mg) 3 to 4 times daily. 
  • Adults: 4-8 teaspoonfuls (480 mg - 960 mg) 3 to 4 times daily. 

(Note: 1 teaspoonful is typically 5 ml) 

Side Effects:

Paracetamol is generally safe at recommended doses. Side effects are rare and usually mild: 

  • Common: Gastrointestinal disturbances (e.g., nausea, vomiting, abdominal pain). 
  • Rare but serious: Blood disorders (e.g., thrombocytopenia, leucopenia, agranulocytosis), pancreatitis, severe skin reactions (e.g., Stevens-Johnson syndrome, toxic epidermal necrolysis), and potentially fatal liver damage (especially with overdose or in patients with pre-existing liver conditions). Allergic reactions (e.g., rash, angioedema) can also occur. 

Contraindications:

Napa Syrup is contraindicated in: 

  • Patients with known hypersensitivity to Paracetamol or any component of the syrup. 
  • Patients with severe hepatic (liver) function impairment. 

Precautions and Warnings:

  • Overdose: Exceeding the recommended dose can cause severe and potentially fatal liver damage. Keep out of reach of children. In case of suspected overdose, seek immediate medical attention, even if symptoms are not apparent. 
  • Liver/Kidney Disease: Use with caution in patients with impaired liver or kidney function. 
  • Concurrent Medications: Avoid taking other medications containing Paracetamol simultaneously to prevent accidental overdose. 
  • Alcohol: Chronic excessive alcohol use increases the risk of liver damage from Paracetamol. 

Pregnancy and Lactation:

  • Pregnancy: Paracetamol is widely used and generally considered safe during pregnancy at recommended doses. However, always consult a doctor before use. 
  • Lactation: Paracetamol is excreted in breast milk in small amounts but is generally considered safe for use during breastfeeding. Consult a doctor. 

Drug Interactions:

  • Warfarin: Prolonged regular use of Paracetamol may enhance the anticoagulant effect of warfarin, increasing bleeding risk. 
  • Certain anticonvulsants (e.g., Phenytoin, Carbamazepine, Barbiturates) and Rifampicin: May increase the risk of Paracetamol-induced liver toxicity with prolonged co-administration. 
  • Metoclopramide or Domperidone: May increase the absorption rate of Paracetamol. 
  • Cholestyramine: May reduce Paracetamol absorption if taken within one hour. 

Storage Conditions:

Store below 30°C in a dry place, protected from light. Keep out of reach of children.
