NFT contains Nitrofurantoin (available as macrocrystals or monohydrate), a highly focused antibacterial drug synthesized specifically to treat and prevent urinary tract infections (UTIs). Once absorbed, it is heavily concentrated in the urine, giving it a characteristic brown color. It works by entering bacterial cells and damaging their ribosomal proteins, DNA, and RNA, effectively stopping bacterial growth.
NFT is indicated for the treatment and long-term prevention (prophylaxis) of uncomplicated UTIs caused by susceptible strains of bacteria, including Escherichia coli, Enterococci, Staphylococcus aureus, Staphylococcus saprophyticus, and certain strains of Klebsiella and Enterobacter.
To minimize stomach upset, it is highly recommended to take Nitrofurantoin with food or milk.
Acute Uncomplicated UTIs: 50 mg to 100 mg four times daily for 7 days.
Long-Term Suppression: 50 mg to 100 mg once daily taken right at bedtime.
Surgical Prophylaxis: 50 mg four times daily for the duration of the procedure and continued for 3 days after.
Acute UTIs: One 100 mg SR capsule every 12 hours for 7 days.
Surgical Prophylaxis: One 100 mg SR capsule twice daily on the day of the procedure and for the next 3 days.
Note: Strictly contraindicated for infants under one month of age.
Active Infection: 5 to 7 mg/kg/day divided across 4 doses daily for 7 days.
7 to 11 kg: 2.5 mL (½ tsp) 4 times daily.
12 to 21 kg: 5 mL (1 tsp) 4 times daily.
22 to 30 kg: 7.5 mL (1½ tsp) 4 times daily.
31 to 41 kg: 10 mL (2 tsp) 4 times daily.
Long-Term Suppression: Doses can be reduced down to 1 mg/kg/day given in 1 or 2 divided doses.
Pulmonary Reactions Alert: Nitrofurantoin can occasionally trigger acute, sub-acute, or chronic lung reactions (manifesting as sudden breathing difficulties, coughing, wheezing, or fever). If any unusual lung symptoms occur, stop taking NFT immediately and seek emergency medical care.
Magnesium Antacid Interaction: Do not take antacids containing Magnesium Trisilicate while using NFT. Magnesium binds to the antibiotic in the digestive tract, drastically reducing its absorption and rendering the treatment ineffective.
Uricosuric Interaction: Gout medications like Probenecid can block your kidneys from processing Nitrofurantoin properly, reducing its concentration in the urine (where it is needed) and increasing drug toxicity in the blood.
NFT is strictly contraindicated in:
Patients with significant kidney impairment, low urine output (oliguria), or an inability to produce urine (anuria).
Pregnant patients at 38–42 weeks (full term), during labor, and delivery. It risks inducing hemolytic anemia in the newborn.
Infants under one month old.
Pregnancy (Category B): Can be used during early to mid-pregnancy if clearly needed, but is strictly banned at full term (weeks 38–42) and during labor.
Lactation: Trace amounts pass into breast milk. Do not use this medication if nursing an infant under one month old due to the risk of serious blood complications in the newborn.
