Indications:Nobesit XR (metformin hydrochloride) is indicated as first-line therapy for the treatment of type 2 diabetes mellitus in adults, especially in overweight and obese patients, when diet and exercise alone have not been sufficient for adequate glycemic control.Metformin works by reducing hepatic glucose production, decreasing intestinal absorption of glucose, and improving insulin sensitivity.It is also used in combination with other oral antidiabetic agents or insulin when adequate glycemic control is not achieved with metformin alone.
Dosage and Administration: The dosage of Nobesit XR should be individualized based on the patient's blood glucose levels and tolerability. It is usually taken orally once daily with the evening meal. The starting dose is typically 500 mg or 750 mg once daily, and the dose may be increased gradually in increments of 500 mg weekly or 750 mg every two weeks, up to a maximum of 2000 mg once daily. Some patients may benefit from 1000 mg twice daily. Tablets should be swallowed whole and not crushed or chewed.Renal function should be assessed before initiating Nobesit XR and periodically thereafter.
Side Effects:Common side effects of metformin include gastrointestinal disturbances such as nausea, vomiting, diarrhea, abdominal pain, and loss of appetite.These side effects are often transient and occur at the beginning of treatment. Taking metformin with food can help reduce these effects.Lactic acidosis is a rare but serious potential side effect.Long-term metformin use may be associated with vitamin B12 deficiency.
Precautions:Nobesit XR is contraindicated in patients with severe renal impairment (eGFR < 30 mL/min/1.73 m²), acute or chronic metabolic acidosis (including diabetic ketoacidosis), and known hypersensitivity to metformin.It should be used with caution in patients with moderate renal impairment, liver disease, heart failure, and in elderly patients. Excessive alcohol intake should be avoided due to the increased risk of lactic acidosis.Temporary discontinuation of metformin may be necessary for surgical procedures or iodinated contrast studies.
