Indications: Nomopil (repaglinide) is an oral blood glucose-lowering drug of the meglitinide class. It is indicated as an adjunct to diet and exercise to lower blood glucose in adults with type 2 diabetes mellitus whose hyperglycemia cannot be controlled satisfactorily by diet and exercise alone. Nomopil is also indicated for use in combination with metformin to lower blood glucose in patients whose hyperglycemia cannot be controlled by exercise, diet,and either Nomopil or metformin alone.
Dosage and Administration: Nomopil should be taken just before or up to 30 minutes before each meal. The dosage is individualized and depends on the patient's blood glucose levels. For patients not previously treated or whose HbA1c is < 8%, the starting dose is 0.5 mg before each meal. For patients previously treated with blood glucose-lowering drugs and whose HbA1c is > 8%, the initial dose is 1 or 2 mg before each meal.Dosage adjustments should be made according to blood glucose response, usually at intervals of 1-2 weeks. The dose may be increased up to 4 mg as a single dose, with a maximum daily dose of 16 mg. If a meal is skipped, the corresponding dose of Nomopil should also be skipped.
Side Effects: The most common side effect of Nomopil is hypoglycemia (low blood sugar). Symptoms may include sweating, paleness, tremor, anxiety, unusual tiredness or weakness, confusion, difficulty concentrating, hunger, and visual disturbances. Other possible side effects include upper respiratory tract infections, diarrhea, constipation, nausea, and vomiting. Hypersensitivity reactions such as rashes and urticaria may also occur.
Precautions: Nomopil is contraindicated in patients with diabetic ketoacidosis, with or without coma, and in patients with type 1 diabetes mellitus. It is also contraindicated in patients with known hypersensitivity to repaglinide or any of its inactive ingredients. Caution should be exercised when Nomopil is used in patients with impaired renal or hepatic function. Alcohol consumption should be avoided as it can increase the risk of hypoglycemia. During periods of stress, such as fever, trauma, or surgery, a temporary change to insulin therapy may be necessary.
Storage: Store below 30°C, protected from light and moisture. Keep out of reach of children.
