Nutrimin-D IV 

Presentation: Sterile, aqueous solution containing amino acids (7%), glucose (10%), and electrolytes. Available in 500 ml glass bottles, manufactured by Beximco Pharmaceuticals Ltd. in Bangladesh.

Indications: Source of amino acids, glucose, and electrolytes for intravenous nutrition in adults and children needing IV nutrition, especially those with basal amino acid requirements.

Dosage and Administration: Individualized based on patient condition, nutritional status, and metabolic stress.

• Adults: Nitrogen requirement 0.10-0.25 g/kg/day, corresponding to 11-27 ml/kg/day. Max 1000-2000 ml per 24 hours, not exceeding 1000 ml in 6 hours (approx. 2.8 ml/min). Base obese patient dose on ideal weight.
• Children and Infants: Max 30 ml/kg/day, with gradual increase in the first week.

Infuse slowly under medical supervision.

Side Effects: Nausea (rare), vomiting, flushing, sweating (with high infusion rates). Transient increase in liver function tests. Possible hypersensitivity and thrombophlebitis (peripheral vein). Hyperphenylalaninemia in severely ill, premature infants.

Precautions: Contraindicated in inborn errors of amino acid metabolism, severe liver dysfunction, severe uremia without dialysis, hyperosmolar nonketotic diabetic coma.

Caution in diabetes mellitus, severe heart failure, renal impairment with fluid restriction/oliguria/anuria. Monitor serum potassium and blood glucose. May increase urinary excretion of copper and zinc. May need exogenous insulin in hyperglycemia. Use cautiously in severe malnutrition (refeeding syndrome risk). Do not use if solution is turbid or contains particles. Discard unused portion.

Storage: Protect from light, store below 25°C. Avoid freezing. Keep out of reach of children.
