Oriham Capsule - (10mg) contains Atomoxetine Hydrochlorideequivalent to Atomoxetine 10 mg. 
It is manufactured by Incepta Pharmaceuticals Ltd.

Indications:Oriham is indicated for the treatment of Attention-Deficit/Hyperactivity Disorder (ADHD)as defined by DSM-IV criteria in children 6 years of age and older, adolescents, and adults.

Pharmacology:The precise mechanism by which Atomoxetine produces its therapeutic effects in ADHD is unknown, but it is thought to be related to the selective inhibition of the pre-synaptic norepinephrine transporter.

Dosage and Administration: The dosage is individualized and should be administered as advised by the physician. Some general guidelines:

• Children and Adolescents up to 70 kg Body Weight: Initiate at a total daily dose of approximately 0.5 mg/kg and increase after a minimum of three days to a target total daily dose of approximately 1.2 mg/kg, administered either as a single daily dose in the morning or as evenly divided doses inthe morning and late afternoon/early evening. The total daily dose may be increased to a maximum of 1.4 mg/kg after an additional two to four weeks in patients who have not achieved an optimal response. The maximum recommended total daily dose is 1.4 mg/kg or 100 mg, whichever is less. 
• Children and Adolescents over 70 kg Body Weight and Adults: Initiate at a total daily dose of 40 mg and increase after a minimum of three days to a target total daily dose of approximately 80 mg administered either as a single daily dose in the morning or as evenly divided doses in the morning and late afternoon/earlyevening.After an additional two to four weeks, the dose may be increased to a maximum of100 mg in patients who have not achieved an optimal response.The maximum recommended total dailydose is 100 mg.  

Oriham can be taken orally with or without food.

Contraindications: Oriham is contraindicated in patients with:

• Known hypersensitivity to atomoxetine.
• Use with or within 2 weeks after discontinuing a monoamine oxidase inhibitor (MAOI).
• Narrow-angle glaucoma.  
• Pheochromocytoma.
• Severe Cardiovascular Disorders.  
• Symptomatic cardiovascular diseases.
• Uncontrolled hyperthyroidism.  

Precautions:

• Suicidal Ideation and Behavior:Monitor patients closely for clinical worsening, suicidality, and unusual changes in behavior, especially during the initialmonths of therapy and when the dose is adjusted.  
• Cardiovascular Effects:Can affect heart rate and blood pressure. Measure heart rate and blood pressure before starting treatment and periodically during treatment. Use cautiously in patients with hypertension, tachycardia, or cardiovascular or cerebrovascular disease. 
• Aggressive Behavior or Hostility: Has been reported more frequently in children and adolescents treated with atomoxetine compared to placebo. Monitor for the appearance or worsening of aggressive behavior or hostility.
• Possible Allergic Reactions: Anaphylactic reactions, rash, angioneurotic edema, and urticaria have been reported.
• Use with caution in patients with a history of seizures, severe liver injury, psychotic or manic symptoms, bipolar disorder.  

Side Effects: Very common side effects include increased blood pressure and increased heart rate. Common side effects may include:

• Abdominal pain  
• Decreased appetite  
• Nausea
• Vomiting
• Fatigue
• Somnolence
• Dizziness
• Irritability
• Rash
• Dyspepsia
• Early morning awakening  
• Ejaculation disorder  
• Ejaculation failure
• Lethargy  
• Tachycardia  
• Paraesthesia  
• Chills

Drug Interactions:

• MAO Inhibitors: Should not be used with MAOIs or within two weeks after discontinuing MAOIs.
• CYP2D6 Inhibitors:Concomitant use may increase atomoxetine steady-state plasma concentrations. Slower titration of Oriham may be necessary. 
• Anti-hypertensive drugs and Vasopressor Agents:Use cautiously due to possible effects on blood pressure.  
• Tricyclic Antidepressants:Can increase the adverse cardiovascular effects of tricyclic antidepressants if co-administered.  

Pregnancy and Lactation:Pregnancy Category B3. Should not be used during pregnancy unless the potential benefit justifies the potential risk to the fetus.It is unknown if atomoxetine is excreted in human milk. Caution should be exercised when administered to a nursing woman.

Storage: Store below 30°C, away from light and moisture. Keep out of reach of children.
