Pantotop 20 

Presentation: Pantotop 20 Tablet, available in Bangladesh, contains Pantoprazole Sodium equivalent to Pantoprazole 20 mg. It is presented as enteric-coated tablets for oral administration, usually available in blister packs.

Indications: Pantotop (Pantoprazole) is indicated for the treatment of acid-related disorders, including:

• Gastro-esophageal reflux disease (GERD), including symptomatic relief.
• Maintenance of healing of erosive esophagitis and reduction in the rate of relapse of heartburn symptoms in patients with GERD.
• Gastric ulcer.
• Duodenal ulcer.
• Prevention of NSAID-induced gastric ulcers and duodenal ulcers in patients at risk who require continued NSAID therapy.
• Eradication of Helicobacter pylori infection (in combination with appropriate antibiotics).  
• Zollinger-Ellison syndrome and other pathological hypersecretory conditions.  

Dosage and Administration: Pantotop 20 tablets should be swallowed whole and should not be chewed or crushed. It is recommended to take Pantotop on an empty stomach, at least 30 minutes to one hour before a meal, preferably in the morning. The dosage will vary depending on the condition being treated and the patient's response. Always follow your doctor's specific instructions.

• GERD: The usual dose is 20 mg once daily for 4 to 8 weeks. For maintenance therapy, 20 mg once daily may be used.
• Gastric Ulcer: The usual dose is 40 mg once daily for 4 to 8 weeks. A higher strength like Pantotop 40 might be prescribed for this.
• Duodenal Ulcer: The usual dose is 40 mg once daily for 2 to 4 weeks. A higher strength like Pantotop 40 might be prescribed for this.
• Prevention of NSAID-induced ulcers: 20 mg once daily.
• H. pylori Eradication: Pantoprazole 40 mg is typically given twice daily in combination with specific antibiotics for a duration determined by the physician. A higher strength like Pantotop 40 is usually used for this.
• Zollinger-Ellison Syndrome: The starting dose is usually 80 mg once daily, which may be adjusted based on acid secretion measurements. Higher strengths like Pantotop 40 are typically used.

Side Effects: Pantotop (Pantoprazole) is generally well-tolerated. Common side effects may include headache, diarrhea, nausea, vomiting, abdominal pain, flatulence, and dizziness. Less common side effects may include dry mouth, skin rash, pruritus, fatigue, and arthralgia. Long-term use may potentially lead to vitamin B12 deficiency or an increased risk of bone fractures in some individuals.

Precautions: Pantotop (Pantoprazole) is contraindicated in patients with known hypersensitivity to pantoprazole or any other components of the formulation. Exercise caution in patients with severe liver disease and during pregnancy and breastfeeding; consult your doctor before use. Inform your doctor about all other medications you are taking, as Pantoprazole can interact with certain drugs. When gastric ulcer is suspected, the possibility of gastric malignancy should be excluded before initiating therapy.

Storage: Store below 30°C, protected from light and moisture. Keep out of reach of children.
