Perosa (Permethrin) Cream: A Parasiticidal Preparation

Perosa cream contains Permethrin, a topical scabicidal agent. It is indicated for the treatment of infestations with scabies (Sarcoptes Scabiei) and crab lice (Pediculosis). Scabies is a contagious condition, and if one family member is infected, all members should be treated together to prevent re-infestation. Perosa cream belongs to the therapeutic class of parasiticidal preparations.


How Perosa Cream Works (Pharmacology)

Permethrin works by disrupting the nervous system of the parasites:

  • Disrupts Sodium Channels: It acts on the nerve cell membranes of the scabies mites and lice to disrupt the flow of sodium ions.
  • Causes Paralysis: This disruption of the sodium channel current leads to delayed repolarization and paralysis of the pests, which ultimately leads to their death.
  • Minimal Systemic Absorption: Less than 0.5% of the applied permethrin is absorbed into the bloodstream. The absorbed amount is rapidly metabolized and excreted, primarily in the urine.

Key Indications & Benefits

Perosa cream is indicated for the treatment of:

  • Scabies infestations.

Dosage & Administration

Perosa cream is applied topically to the skin. The recommended dosage and application method vary by age. Always consult a registered physician for specific dosage instructions.

  • Before application: The skin should be clean, cool, and dry. Avoid a hot shower or bath immediately before applying.
  • Adults and children over 2 years: Apply the cream to the entire body from the neck down, rubbing it lightly into the skin. This includes areas like between the fingers and toes, under the nails, and on the soles of the feet.
  • Babies under 2 years: Apply to the face, neck, ears, and scalp as well, avoiding the immediate area around the eyes and mouth.
  • Leave on for at least 8 hours: The cream should be left on for a minimum of 8 hours before being washed off. If any area is washed during this time (e.g., hands), the cream should be reapplied to that spot.
  • Re-treatment: If necessary, a second application can be performed after 7 days, as advised by a doctor.

Important Considerations & Warnings

It is crucial to discuss your full medical history with your doctor before using Perosa cream.

Contraindications:

  • Known hypersensitivity to permethrin, other pyrethroids, or any component of the cream.

Side Effects:

  • Common: Skin discomfort, described as burning, stinging, or tingling, which may occur soon after application.
  • Other temporary signs of irritation: Redness (erythema), swelling (edema), eczema, rash, and itching (pruritus).

Pregnancy & Lactation:

  • Pregnancy: It should only be used during pregnancy if medically advised, as there are no specific studies on its use in pregnant women.
  • Lactation: It is not known whether permethrin is excreted in human milk. Therefore, it should not be used by nursing mothers.

Precautions & Warnings:

  • Exacerbation of Symptoms: Scabies symptoms like itching, redness, and swelling may temporarily worsen after treatment with Perosa.
  • Eye Contact: Avoid contact with the eyes during application. If the cream gets in the eyes, flush immediately with water.
  • Gloves: Nursing staff who routinely apply the cream should wear gloves to prevent possible hand irritation.
  • Corticosteroids: Treatment of eczematous-like reactions with corticosteroids should be withheld prior to Perosa treatment, as there is a risk of exacerbating the scabies by reducing the immune response to the mite.

Storage Conditions

Store in a cool, dry place, away from light and out of the reach of children.
