Phosphoprep 

Presentation: Phosphoprep is an oral solution containing Sodium Dihydrogen Phosphate Dihydrate and Disodium Hydrogen Phosphate Dodecahydrate as the active ingredients. The concentration of these ingredients is typically 24.4 g and 10.8 g per 100 ml, respectively. Phosphoprep is a product marketed by various pharmaceutical companies and is usually available in bottles containing a specific volume of the solution (e.g., 45 ml). It is often available in a pack containing two bottles, as the preparation for colonoscopy usually requires two doses.

Indications: Phosphoprep is indicated for bowel cleansing prior to colonoscopy or other clinical procedures requiring a clean colon. It works by increasing the fluid volume in the small intestine, leading to watery bowel movements that empty the colon.

Dosage and Administration: The dosage regimen for Phosphoprep usually involves taking two doses, one the evening before the procedure and the second on the morning of the procedure. The exact timing and dosage should be strictly followed as directed by your doctor or the instructions provided with the product.

A typical regimen might be:

  • Evening before the procedure: The contents of one bottle (45 ml) are diluted with water up to a total volume of 120 ml and drunk over a period of 15-30 minutes, followed by drinking at least one litre of clear fluids (water, clear soup, strained fruit juice without pulp, herbal tea, black tea or coffee without milk).
  • Morning of the procedure (at least 4 hours before): The contents of the second bottle (45 ml) are diluted with water up to a total volume of 120 ml and drunk over a period of 15-30 minutes, followed by drinking at least 750 ml of clear fluids.

Ensure you drink the recommended amount of clear fluids to avoid dehydration. Do not consume any solid food from the start of the preparation until after the procedure.

Side Effects: Phosphoprep can cause significant bowel activity, leading to:

  • Abdominal bloating and distension
  • Abdominal pain or cramping
  • Nausea
  • Vomiting
  • Diarrhoea (frequent and watery stools are expected)

Less common side effects may include dizziness, headache, and electrolyte imbalances (such as hyperphosphataemia, hypokalaemia, hypocalcaemia). Dehydration is a potential risk if adequate fluid intake is not maintained. In rare cases, more serious side effects like cardiac arrhythmias or seizures may occur, especially in individuals with pre-existing conditions.

Precautions: Phosphoprep is contraindicated in patients with:

  • Known hypersensitivity to sodium phosphates or any of the excipients.
  • Bowel obstruction or ileus.
  • Gastric retention.
  • Bowel perforation.
  • Toxic colitis or toxic megacolon.
  • Active inflammatory bowel disease (e.g., Crohn's disease, ulcerative colitis).
  • Congestive heart failure.
  • Severe renal impairment.
  • Uncontrolled hypertension.
  • Ascites.
  • Known or suspected electrolyte disturbances (e.g., dehydration, hypernatraemia, hyperphosphataemia).

Use with caution in elderly or debilitated patients and those with controlled hypertension or cardiac disease. Ensure adequate hydration is maintained. Phosphoprep can interact with certain medications; inform your doctor of all medications you are taking, especially diuretics, ACE inhibitors, angiotensin receptor blockers, and NSAIDs, as they may increase the risk of electrolyte imbalances or renal problems.

Use in Pregnancy and Lactation: There is limited data on the use of Phosphoprep in pregnant women. It should be used during pregnancy only if the potential benefit justifies the potential risk to the foetus. It is not known whetherPhosphoprep is excreted in human milk. Caution should be exercised when administering Phosphoprep to breastfeeding women.

Storage: Store below 25°C. Do not refrigerate or freeze. Keep out of reach of children.
