Povisep (Povidone Iodine): A Broad-Spectrum Antiseptic
Povisep contains Povidone Iodine, a complex of iodine and an organic polymer. It is a broad-spectrum microbicide with rapid germicidal activity against a wide range of microorganisms, including bacteria, protozoa, viruses, and fungi. Povisep is non-staining and its activity is not diminished by the presence of soap, blood, or pus. It is available in various forms, including cream, ointment, solution, gargle, surgical scrub, powder, and ophthalmic solution. Povisep belongs to the therapeutic class of iodine compounds (anti-septic preparations).
How Povisep Works (Pharmacology)
Povidone Iodine's mechanism of action is due to the slow and sustained release of elemental iodine:
- Slow Release of Iodine: The povidone polymer slowly liberates elemental iodine, which is responsible for the germicidal activity.
- Broad-Spectrum Activity: This mechanism provides rapid microbicidal action against both Gram-positive and Gram-negative bacteria, as well as protozoa, viruses, and fungi. It is also sporicidal.
- Lower Toxicity: The slow-release nature makes Povidone Iodine less toxic than ordinary elemental iodine.

Povisep is indicated for:
- Cream/Ointment: Treatment of primary or secondary topical infections, infected surgical incisions, burns, and other skin lesions.
- Solution: Prevention and treatment of surface infections, preoperative and postoperative skin preparation, and disinfection of wounds.
- Gargle & Mouthwash: Treatment of acute mucosal infections of the mouth and pharynx, and for oral hygiene.
- Surgical Scrub: Antiseptic skin cleansing for surgeons and for preoperative preparation of patients' skin.
- Powder: Topical application for superficial wounds, minor cuts, burns, and to prevent infection.
- Ophthalmic Solution: Symptomatic treatment of dry eye conditions.
Dosage & Administration
The dosage and method of administration depend on the specific formulation being used. Always consult a registered physician, dentist, or surgeon for specific instructions.
- Cream/Ointment: Apply once or twice daily for a maximum of 14 days for treatment.
- Gargle & Mouthwash: Use up to 10 mL undiluted or diluted with warm water, gargle for up to 30 seconds, up to four times daily. Do not swallow.
- Surgical Scrub: Used for scrubbing hands and forearms of operating personnel and for preparing a patient's skin before surgery.
- Powder: Apply a light dusting to the affected area.
- Ophthalmic Solution: Instill 1 drop into the conjunctival sac 4 times daily or as required.

Contraindications:
- Hypersensitivity: Patients with known hypersensitivity to povidone iodine or iodine.
- Thyroid disorders: Regular use is to be avoided in patients with thyroid disorders (nodular colloid goiter, endemic goiter, Hashimoto's thyroiditis) or those on lithium therapy.
- Children: Povidone Iodine Powder should not be used in children under 2 years of age.
Side Effects:
- Hypersensitivity reactions and skin/mucous membrane irritation.
- Application to severe burns or large areas may lead to systemic effects like metabolic acidosis and renal impairment.
- It may interfere with thyroid function tests.
Pregnancy & Lactation:
- Pregnancy: Not recommended for use during pregnancy, especially with regular use of gargle, scrub, or topical preparations, due to the possibility of iodine absorption affecting the fetal thyroid.
- Lactation: Regular use of gargle, scrub, or topical products should be avoided. The ophthalmic solution is also not recommended unless compelling reasons exist.
Precautions & Warnings:
- Deep Wounds/Serious Burns: For deep wounds or serious burns, a physician should be consulted.
- Pooling: Avoid "pooling" of the solution beneath the patient during preoperative prepping to prevent irritation.
- Mouthwash: Do not use the gargle for more than 14 consecutive days.
- White Gold Jewelry: The surgical scrub can permanently discolor white gold jewelry.
- Ophthalmic Solution: Do not wear contact lenses during instillation. Do not touch the dropper tip to any surface to avoid contamination. Discard after one month of opening.
Drug Interactions:
- It interacts with strong alkali, sodium thiosulphate, sodium metabisulphite, and thiomersal.
- Use with concurrent lithium therapy may lead to a hypothyroidic effect.
Storage Conditions
Store below 25°C in a cool and dry place, protected from light. Do not freeze. Keep out of the reach of children. The ophthalmic solution should be discarded one month after opening.
