Prosol IV
Presentation: Sterile, aqueous solution containing crystalline amino acids and D-sorbitol with electrolytes. Available in 500 ml bottles for intravenous infusion. Manufactured by Incepta Pharmaceuticals Ltd.
Indications: Indicated as a source of amino acids for protein synthesis in patients needing intravenous nutrition, particularly those with basal amino acid requirements. Also used for faster recovery in surgery, burns, renal insufficiency, hepatic insufficiency, and in the management of cancer.
Dosage and Administration: Dosage is individualized based on the patient's condition, nutritional status, body weight, and metabolic needs. The nitrogen requirement for maintenance typically ranges from 0.10-0.25 g nitrogen/kg/day, corresponding to 15-35 ml/kg/day of the 5% solution. Infuse slowly, usually at a rate of 1.4-2.8 ml/min (30-60 drops/min). In obese patients, the dose should be based on ideal body weight. In infants and children, the rate of infusion is typically 28-35 ml/kg body weight per day, with a gradual increase during the first week. Administer intravenously under strict medical supervision. Prosol is for admixing only and must be diluted before infusion.
Side Effects: Generally well-tolerated. Nausea occurs rarely. Possible vomiting, flushing, and sweating may occur if infused too rapidly. Transient increases in liver function tests and hypersensitivity reactions have been reported. Thrombophlebitis may occur with peripheral vein infusion. Hyperphenylalaninemia may occur in severely ill, premature infants. Increased urinary excretion of zinc and copper may occur during long-term use.
Precautions: Contraindicated in patients with inborn errors of amino acid metabolism, irreversible liver damage, and severe uremia without dialysis. Due to the potential glucose content in some formulations, it may be contraindicated in hyperosmolar nonketotic diabetic coma. Use with caution in patients with diabetes mellitus, severe heart failure, or renal impairment with fluid restriction or oliguria/anuria. Monitor serum potassium and blood glucose regularly. Ensure adequate phosphate supply in hypophosphatemia. Do not use if the solution is turbid or contains particles.
Storage: Protect from light and store between 15°C to 25°C. Avoid freezing. Keep out of reach of children. Discard any unused portion.
