Protium 

Presentation:Protium is available as gastro-resistant tablets containing Pantoprazole Sodium Sesquihydrate equivalent to Pantoprazole.It typically comes in strengths of 20 mg and 40 mg. The tablets are usually oval, biconvex, and film-coated, often with an imprint indicating the strength (e.g., "P20" or "P40"). Protium is a product of Unimed Unihealth MFG.Ltd. in Bangladesh. It is usually available in blister packs.

Indications: Protium (Pantoprazole) is a proton pump inhibitor (PPI) indicated for conditions where the reduction of gastric acid secretion is beneficial, including:

  • Gastroesophageal Reflux Disease (GERD), including symptomatic relief and long-term management of reflux oesophagitis. 
  • Peptic ulcer diseases (gastric and duodenal ulcers). 
  • Eradication of Helicobacter pyloriin combination with appropriate antibiotics. 
  • Prevention of gastroduodenal ulcers induced by non-selective non-steroidal anti-inflammatory drugs (NSAIDs) in patients atrisk requiring continuous NSAID treatment. 
  • Zollinger-Ellison Syndrome and other pathological hypersecretory conditions. 

Dosage and Administration:Protium tablets should be swallowed whole with water, one hour before a meal, preferably in the morning.Do not chew or crush the tablets.The dosage and duration of treatment depend on the condition being treated and the patient's response.

  • GERD: The usual dose is 20-40 mg daily for 4-8 weeks, with a maintenance dose of 20-40 mg daily if needed. For symptomatic relief, 20 mg once daily may be used.
  • Peptic Ulcer: The usual dose is 40 mg daily in the morning for 2-4 weeks for duodenal ulcers and 4-8 weeks for benign gastric ulcers.
  • H. pyloriEradication:Protium 40 mg is typically given twice daily in combination with antibiotics for one week. 
  • NSAID-induced Ulcer Prophylaxis: 20 mg once daily.
  • Zollinger-Ellison Syndrome: The initial dose is usually 80 mg once daily, adjusted based on acid secretion. Daily doses above 80 mg may be given in two divided doses. The maximum daily dose in the elderly is usually 40 mg.

Side Effects:Protium (Pantoprazole) is generally well-tolerated.Common side effects may include:

  • Headache
  • Diarrhoea
  • Abdominal pain
  • Nausea 
  • Vomiting 
  • Flatulence

Less common side effects may include dizziness, sleep disorders, rash, pruritus (itching), and fatigue. Long-term use may potentially lead to an increased risk of bone fractures, vitamin B12 deficiency, and hypomagnesaemia (low magnesium levels).

Precautions:Protium (Pantoprazole) is contraindicated in patients with known hypersensitivity to pantoprazole or any other component of the formulation. 

Caution should be exercised in patients with:

  • Severe liver impairment (the daily dose should usually not exceed 20 mg).
  • Renal impairment (no dose adjustment is usually necessary).
  • A history of allergic reactions to other proton pump inhibitors.
  • Risk factors for osteoporosis (long-term use may increase fracture risk).
  • Potential for vitamin B12 deficiency with prolonged use. 

Inform your doctor about all other medications you are taking, as Pantoprazole can interact with certain drugs, including warfarin, ketoconazole, itraconazole, and medications for HIV. 

Use in Pregnancy and Lactation: Pantoprazole is classified as Pregnancy Category B by the US FDA. While animal studies have not shown teratogenic effects, there are no adequate and well-controlled studies in pregnant women. Protium should be used during pregnancy only if clearly needed. Pantoprazole has been shown to be excreted in human milk. A decision should be made whether to discontinue nursing or to discontinue the drug, taking into account thebenefit of the drug to the mother. 

Storage:Store below 30°C, protected from light and moisture.Keep out of reach of children. Do not use after the expiry date.
