Quiet XR 50 contains Quetiapine Fumarate, an atypical psychotropic agent (atypical neuroleptic). It works by balancing multiple neurotransmitters in the brain—specifically serotonin and dopamine—to regulate mood, thinking, and behavior.
Quiet XR 50 is clinically indicated for the treatment and management of:
Schizophrenia: Long-term management of acute and chronic psychoses.
Bipolar Mania: Acute treatment of moderate-to-severe manic episodes associated with bipolar disorder.
Bipolar Depression: Direct targeted relief for depressive episodes linked to bipolar disorder.
Bipolar Maintenance: Used alongside mood stabilizers to prevent the recurrence of manic, mixed, or depressive episodes.
Because this medication influences brain chemistry, doses are stepped up gradually (titrated) to build tolerance.
Initial Step-Up: Day 1: 50 mg ➡️ Day 2: 100 mg ➡️ Day 3: 200 mg ➡️ Day 4: 300 mg.
Maintenance Dose: The standard effective range is 300 mg to 450 mg/day split into two doses, adjusted based on patient response.
Initial Step-Up: Day 1: 100 mg ➡️ Day 2: 200 mg ➡️ Day 3: 300 mg ➡️ Day 4: 400 mg.
Maintenance Dose: Titrated up to a usual effective range of 400 mg to 800 mg/day.
Initial Step-Up: Day 1: 50 mg ➡️ Day 2: 100 mg ➡️ Day 3: 200 mg ➡️ Day 4: 300 mg.
Administration Note: Taken strictly once daily at bedtime due to its prominent sedating effects.
Suicidal Thoughts: Patients with depression may experience a temporary increase in suicidal ideation during the initial stages of therapy. Close monitoring is essential.
Orthostatic Hypotension: Quetiapine can cause a sharp drop in blood pressure when standing up, leading to dizziness or fainting, especially during the first few days of titration.
Metabolic Changes: Long-term use can alter baseline metabolic markers, leading to weight gain, elevated blood glucose (hyperglycemia), and increased cholesterol or triglyceride levels.
Movement Disorders: Discontinue use and seek evaluation if signs of Tardive Dyskinesia (uncontrolled, rhythmic muscle movements) or Neuroleptic Malignant Syndrome (high fever, muscle stiffness) occur.
Do Not Use If: You have a known hypersensitivity to Quetiapine Fumarate.
Clearance Interactions: Anti-seizure medications like Phenytoin and Carbamazepine significantly increase the clearance of Quetiapine, reducing its effectiveness.
Cardiac Warning: Use extreme caution when combining with medications that prolong the QT interval or cause electrolyte imbalances.
Pregnancy: Safety profiles are not fully established; use only if potential benefits clearly outweigh the risks to the fetus.
Lactation: It is advised to avoid breastfeeding while taking this medication as excretion rates into breast milk remain unknown.
Store in a cool, dry place away from moisture and direct light.
