Rabe 20 (Rabeprazole 20 mg): A Proton Pump Inhibitor (PPI)

Rabe 20 contains Rabeprazole Sodium 20 mg, a proton pump inhibitor (PPI) that reduces gastric acid secretion. It is used to treat various conditions caused by excessive stomach acid.


How Rabe 20 Works (Pharmacology)

Rabeprazole works by:

  • Inhibiting Gastric H+/K+-ATPase: It specifically inhibits the gastric H+/K+-ATPase (the proton pump) at the secretory surface of the stomach parietal cells. This enzyme is responsible for pumping hydrogen ions (protons) into the stomach, which is the final step in acid production.
  • Reducing Gastric Acid Secretion: By blocking this "acid pump," Rabeprazole effectively reduces the amount of acid produced in the stomach.

Key Indications & Benefits

Rabe 20 is indicated for:

  • Acidity/Heartburn.
  • Gastroesophageal reflux disease (GERD): Including symptomatic, erosive, or ulcerative GERD.
  • Maintenance of healing of erosive or ulcerative GERD.
  • Peptic ulcer disease: Including active duodenal ulcer and active benign gastric ulcer.
  • Zollinger-Ellison Syndrome: A rare condition causing excessive stomach acid production.
  • Eradication of Helicobacter pylori infection: In combination with appropriate antibacterial therapy (e.g., amoxicillin and clarithromycin) in patients with peptic ulcer disease to reduce the risk of recurrence.
  • NSAID-induced ulcers: Treatment and prophylaxis.

Dosage & Administration

Rabe 20 mg tablets or capsules should be swallowed whole and are generally taken once daily, preferably in the morning, one hour before a meal. However, it can also be taken with or without food. Always consult a registered physician for specific dosage instructions.

Common Dosages:

  • Healing of Erosive or Ulcerative GERD: 20 mg once daily for 4 to 8 weeks. An additional 8-week course may be considered if healing is not complete.
  • Maintenance of Healing of Erosive or Ulcerative GERD: 20 mg once daily.
  • Symptomatic GERD: 20 mg once daily for 4 weeks. An additional course may be considered if symptoms persist.
  • Healing of Duodenal Ulcer: 20 mg once daily (often after the morning meal) for up to 4 weeks.
  • Helicobacter pylori Eradication: 20 mg twice daily, in combination with Amoxicillin (1000 mg twice daily) and Clarithromycin (500 mg twice daily), for 7 days. This combination is usually taken with morning and evening meals.
  • Zollinger-Ellison Syndrome: Starting dose is typically 60 mg once daily. Doses are adjusted individually and can be up to 100 mg once daily or 60 mg twice daily. Treatment continues as long as clinically indicated.

Important Considerations & Warnings

It is crucial to discuss your full medical history with your doctor before taking Rabe 20.

Contraindications:

  • Patients with known hypersensitivity to Rabeprazole or any component of the formulation.

Side Effects:

  • Generally mild and transient.
  • Most common: Headache, diarrhea, nausea, vomiting, abdominal pain, flatulence, dry mouth.
  • Other reported side effects: Weakness, dizziness, skin reactions.
  • Long-term use (more than 1 year) and/or higher doses may increase the risk of:
    • Bone fractures (especially in older adults). Your doctor may recommend calcium and vitamin D supplements.
    • Low blood magnesium levels (hypomagnesemia): Can occur after 3 months or more of use, leading to tiredness, confusion, dizziness, muscle twitches, and irregular heartbeat. Your doctor may monitor magnesium levels.
    • Vitamin B12 deficiency: Symptoms include feeling very tired, sore and red tongue, mouth ulcers, and pins and needles.
    • Gut infections (e.g., Clostridioides difficile).
    • Rarely, severe skin reactions (e.g., SJS, TEN), kidney problems (acute tubulointerstitial nephritis), or types of lupus.

Pregnancy & Lactation:

  • Pregnancy: Rabeprazole may be unsafe to use during pregnancy. Animal studies have shown harmful effects on the developing baby. Consult your doctor to weigh benefits and risks.
  • Lactation: Rabeprazole is probably unsafe to use during breastfeeding. Limited human data suggest it may pass into breast milk and harm the baby. Consult your doctor.

Precautions & Warnings:

  • Masking Malignancy: Symptomatic response to therapy with rabeprazole does not preclude the presence of gastric malignancy. Further investigation may be needed if symptoms persist.
  • Driving/Operating Machinery: Rabeprazole may decrease alertness, affect vision, or cause sleepiness/dizziness. Avoid driving or operating machinery if these symptoms occur.
  • Alcohol: Caution is advised when consuming alcohol.
  • Kidney/Liver Disease: Generally considered safe with dosage adjustments not typically recommended for kidney or liver disease, though caution may be advised for severe cases.

Drug Interactions:

  • Gastric pH-dependent Absorption: Rabeprazole inhibits gastric acid secretion and may interfere with the absorption of drugs where gastric pH is important for bioavailability (e.g., ketoconazole, iron salts, and digoxin).
  • No significant interaction with liquid antacids or foods has been observed in studies.
  • No clinically important interactions identified with specific drugs like Propranolol, Digoxin, Warfarin, Theophylline.

Overdose Effects

  • Limited data on human overdosage.
  • Management should be symptomatic and supportive.
  • Rabeprazole is extensively protein-bound and is not expected to be significantly removed by hemodialysis.

Storage Conditions

Keep all medicines out of reach of children. Store in a cool and dry place, protected from light and moisture.
