Radiglip M 500 

Presentation:Radiglip M 500 Tablet, available in Bangladesh, is a combination tablet containing Sitagliptin 50 mgand Metformin Hydrochloride BP 500 mg. 

Indications:Radiglip M is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellituswhentreatment with both sitagliptin and metforminis appropriate.This includes patients whose type 2 diabetes is not adequately controlled on metformin alone, or who are already being treated with separate tablets of sitagliptin and metformin.

Dosage and Administration:The dosage of Radiglip M should be individualized based on the patient's current treatment regimen, glycemic control, and tolerability, using the lowest effective dose.It is generally administered twice daily with mealsto reduce the gastrointestinal side effects associated with metformin.  

  • Patients not currently treated with metformin: The usual starting dose is one tablet of Radiglip M 500 twice daily with meals.
  • Patients switching from metformin monotherapy:The starting dose of Radiglip M should provide the same total daily dose of metformin as was previously taken, plus sitagliptin 50 mg twice daily. 
  • Patients switching from separate tablets of sitagliptin and metformin: The starting dose of Radiglip M should be based on the doses of sitagliptin and metformin the patient was already taking.
  • Maximum Daily Dose:The maximum recommended daily dose of sitagliptin is 100 mg and metformin is 2000 mg.With Radiglip M 500, the maximum recommended dose is two tablets twice daily (sitagliptin 100 mg and metformin 2000 mg).
  • Administration:The tablet should be swallowed whole and should not be crushed, split, or chewed.  

Side Effects:Common side effects associated with metformin include gastrointestinal disturbances such as diarrhea, nausea, vomiting, andflatulence, particularly during initiation of therapy.Side effects potentially related to sitagliptin include upper respiratory tract infection, nasopharyngitis, and headache.Hypoglycemia is not usually seen with metformin or sitagliptin monotherapy but can occur when Radiglip M is used in combination with insulin secretagogues (e.g., sulfonylureas). Pancreatitis has been reported in some patients receiving sitagliptin.Less common side effects may include constipation, abdominal pain, and peripheral edema. Lactic acidosis is a rare but serious side effect associated with metformin.

Precautions: Radiglip M is contraindicated in patients with renal disease or renal dysfunction (e.g., serum creatinine levels ≥1.5 mg/dL in males, ≥1.4 mg/dL in females, or abnormal creatinine clearance), acute or chronic metabolic acidosis (including diabetic ketoacidosis),or a history of serious hypersensitivity reaction to sitagliptin or metformin. It is not recommended for use in patients with type 1 diabetes mellitus. Caution should be exercised in patients with moderate renal impairment, heart failure, liver disease, and excessive alcohol intake due to the increased risk of lactic acidosis associated with metformin. Renal function should be assessed prior to initiation of therapy and periodically thereafter. Discontinue Radiglip M if pancreatitis is suspected.

Storage:Store Radiglip M 500 tablets in a cool (below 30°C) and dry place, protected from light and moisture.Keep out of reach of children.
