Remicron MR 30 

Presentation:Remicron MR 30 Tablet, available in Bangladesh, contains Gliclazide 30 mgin a modified-release (MR) formulation.

Indications:Remicron MR is indicated for the treatment of type 2 diabetes mellitusin adults when diet, exercise, and weight reduction alone are not sufficient to achieve adequate glycemic control.  

Dosage and Administration: The dosage of Remicron MR must be individualized based on the patient's blood glucose levels. The usual initial dose is 30 mg once daily.

• The tablet should be swallowed whole with a glass of water at breakfast time and preferably at the same time each day. It should not be crushed or chewed.
• It is essential to have regular meals after taking this medication to reduce the risk of hypoglycemia.
• Dosage adjustments are usually made in increments of 30 mg every 2 weeks based on blood glucose response. The maximum recommended daily dose is 120 mg.
• If a dose is missed, the patient should not take a double dose to compensate for the missed one.The next dose should be taken at the usual time.

Side Effects: The most common side effect is hypoglycemia (low blood sugar). Symptoms can include headache, dizziness, sweating, pallor, tremor, anxiety, confusion, palpitations, increased hunger, and in severe cases, loss of consciousness or seizures. Gastrointestinal disturbances such as nausea, vomiting, abdominal pain, and diarrhea may also occur, although they are less frequent with the MR formulation. Less common side effects include skin reactions (rash, itching, urticaria), and changes in blood counts (e.g., thrombocytopenia, leukopenia). In rare cases, liver function abnormalities may occur.  

Precautions: Remicron MR is contraindicated in patients with:

• Known hypersensitivity to gliclazide or other sulfonylureas or sulfonamides. 
• Type 1 diabetes mellitus.
• Diabetic ketoacidosis, diabetic pre-coma and coma.
• Severe renal or hepatic impairment. 
• Treatment with miconazole (systemic route or oromucosal gel).
• Breastfeeding.

It is generally not recommended during pregnancy. Caution should be exercised in patients with moderate renal or hepatic impairment, elderly or malnourished patients, and in conditions where blood glucose control may be unstable (e.g., intercurrent illness, surgery). Alcohol consumption should be avoided as it can increase the risk of hypoglycemia.Certain medications can interact with gliclazide, potentially affecting blood glucose control; therefore, it is important to inform the doctor about all other medications being taken.  

Storage: Store Remicron MR 30 tablets in a cool (below 30°C) and dry place, protected from light and moisture. Keep this medicine out of the reach of children. Do not use after the expiry date stated on the packaging.
