The tablets are film-coated and intended for oral administration. The appearance may vary depending on the manufacturer and what is available in Bangladesh.
Indications: Renocal Plus is indicated for the treatment of hyperphosphatemia (high phosphate levels in the blood) associated with chronic renal insufficiency in patients undergoing dialysis (hemodialysis, peritoneal dialysis). It is indicated for use in adults. The combination of calcium acetate and magnesium carbonate acts as phosphate binders in the gut, leading to the formation of insoluble calcium and magnesium phosphate salts that are excreted in the feces.
Dosage and Administration: The dosage should be individualized and determined by a physician based on the patient's serum phosphate levels. Renocal Plus tablets are for oral administration and should be swallowed whole with or without food. It is important to take Renocal Plus with meals to maximize its phosphate-binding capacity. The tablets should not be chewed or crushed. Regular monitoring of serum phosphate, magnesium, and calcium levels is necessary during treatment.
Side Effects: Common side effects may include gastrointestinal disturbances such as nausea, vomiting, and constipation. High doses and long-term administration may result in hypermagnesemia (high magnesium levels). Symptoms of hypermagnesemia can include gastrointestinal disturbances, muscle weakness, lethargy, and in severe cases, hypotension, respiratory depression, and cardiac arrest. Hypercalcemia (high calcium levels) is also a potential risk, especially with concurrent use of Vitamin D or thiazide diuretics.
Precautions: Renocal Plus is contraindicated in patients with hypercalcemia. Continuous monitoring of serum phosphate, magnesium, and calcium levels, as well as the calcium-phosphate product, is essential, especially with simultaneous intake of Vitamin D preparations and thiazide diuretics. Caution should be exercised in patients on a controlled sodium diet as this product may contain sodium. It can affect the absorption of certain medications, including some antibiotics (tetracyclines, quinolones, cephalosporins), bisphosphonates, fluorides, ketoconazole, estramustine preparations, anticholinergics, zinc, and ursodeoxycholic acid. Separate administration times for these medications and Renocal Plus should be considered. Use during pregnancy and breastfeeding is generally not recommended unless the clinical condition necessitates treatment.
