Reset (Paracetamol/Acetaminophen): A Non-Opioid Analgesic

Reset contains Paracetamol (Acetaminophen), a widely used non-opioid analgesic. It is indicated for the relief of fever and various types of pain, from common aches to post-operative discomfort. It belongs to the therapeutic class of Non-Opioid Analgesics.


How Reset Works (Pharmacology)

Paracetamol's exact mechanism of action is still being fully understood, but it is primarily thought to act in the Central Nervous System (CNS):

  • Analgesic Action: It increases the pain threshold by inhibiting cyclooxygenase (COX) enzymes (COX-1, COX-2, and possibly COX-3) involved in prostaglandin (PG) synthesis, mainly in the CNS. Unlike NSAIDs, it does not significantly inhibit COX in peripheral tissues, which explains its weak anti-inflammatory activity.
  • Antipyretic Action: It produces its fever-reducing effect by inhibiting the hypothalamic heat-regulating center. This leads to peripheral vasodilation and sweating, thus promoting heat loss.
  • Indirect COX Inhibition: Studies suggest Paracetamol indirectly blocks COX, and this blockade is ineffective in the presence of peroxides, which might explain its selective action in the CNS and endothelial cells rather than in platelets and immune cells (which have high peroxide levels).

Key Indications & Benefits

Reset is indicated for:

  • Fever, common cold, and influenza.
  • Various types of pain: Headache, toothache, earache, bodyache, myalgia (muscle pain), neuralgia, dysmenorrhea (menstrual pain), sprains, colic pain, back pain, post-operative pain, postpartum pain, and inflammatory pain.
  • Post-vaccination pain in children.
  • Rheumatic & osteoarthritic pain and stiffness of joints.

Dosage & Administration

Reset comes in various forms (tablet, extended-release tablet, syrup/suspension, suppository, paediatric drop, and IV infusion), and dosages vary significantly by age, weight, and formulation. Always consult a registered physician for specific dosage instructions.

General Guidelines (example dosages; always refer to specific product instructions):

  • Tablet (Standard):
    • Adult: 1-2 tablets every 4-6 hours (max 4 gm/8 tablets daily).
    • Children (6-12 years): ½ to 1 tablet 3-4 times daily (max 2.6 gm/day for long-term).
  • Extended Release Tablet (swallowed whole, not crushed):
    • Adults & Children over 12 years: Two tablets every 6-8 hours (max 6 tablets in 24 hours).
  • Syrup/Suspension:
    • Children under 3 months: 10 mg/kg body weight 3-4 times daily (reduce to 5 mg/kg if jaundiced).
    • 3 months to below 1 year: ½ to 1 teaspoonful 3-4 times daily.
    • 1-5 years: 1-2 teaspoonful 3-4 times daily.
    • 6-12 years: 2-4 teaspoonful 3-4 times daily.
    • Adults: 4-8 teaspoonful 3-4 times daily.
  • Suppository:
    • Children 3-12 months: 60-120 mg, 4 times daily.
    • Children 1-5 years: 125-250 mg 4 times daily.
    • Children 6-12 years: 250-500 mg 4 times daily.
    • Adults & children over 12 years: 0.5-1 gm 4 times daily.
  • Paediatric Drop (e.g., Upto 3 months: 0.5 ml (40 mg); 4-11 months: 1.0 ml (80 mg); 7 months-2 years: 1.5 ml (120 mg)). Do not exceed more than 5 doses daily for a maximum of 5 days.
  • IV Infusion (hospital use, precise dosing by weight and age): Max daily dose 4000 mg for adults.

Important Considerations & Warnings

Contraindications:

  • Known hypersensitivity to Paracetamol.

Side Effects:

  • Side effects are usually mild.
  • Rarely reported: Haematological reactions (thrombocytopenia, leucopenia, pancytopenia, neutropenia, agranulocytosis), pancreatitis, skin rashes, and other allergic reactions.

Pregnancy & Lactation:

  • Pregnancy: Epidemiological studies show no ill effects with recommended dosages, but always follow your doctor's advice.
  • Lactation: Excreted in breast milk in clinically insignificant amounts. Published data do not contraindicate breastfeeding.

Precautions & Warnings:

  • Hepatic Injury/Overdose: Administration of doses higher than recommended can cause hepatic injury, severe hepatotoxicity, and even death.Do not exceed the maximum recommended daily dose.
  • High-Risk Patients for Hepatotoxicity: Use caution in patients with hepatic impairment or active hepatic disease, alcoholism, chronic malnutrition, severe hypovolemia (e.g., due to dehydration or blood loss). The hazard of overdose is greater in those with non-cirrhotic alcoholic liver disease.
  • Renal Impairment: Care is advised in patients with severe renal impairment (creatinine clearance < 30 ml/min).
  • Other Paracetamol-containing products: Patients should be advised not to take other Paracetamol-containing products concurrently to avoid overdose.
  • Hypersensitivity/Allergy: Infrequent reports of life-threatening anaphylaxis. Discontinue immediately if symptoms of allergy or hypersensitivity occur. Do not use if there's a known allergy to Paracetamol.

Drug Interactions:

  • Enzyme Inducers: Patients who have taken barbiturates, tricyclic antidepressants, and alcohol may show a diminished ability to metabolize large doses of Reset, potentially increasing toxicity risk.
  • Alcohol: Can increase the hepatotoxicity of Reset overdosage.
  • Anticonvulsants or Oral Steroid Contraceptives: Chronic ingestion of these drugs can induce liver enzymes, potentially preventing therapeutic Reset levels by increasing its metabolism or clearance.

Use in Special Populations

  • Pediatric Use: Safety and effectiveness of Reset IV for acute pain and fever in pediatric patients 2 years and older are supported by adult studies.
  • Geriatric Use: No overall differences in safety or effectiveness observed between elderly and younger subjects.
  • Patients with Hepatic Impairment: Contraindicated in severe hepatic impairment or severe active liver disease. Use with caution in others. A reduced total daily dose may be warranted.
  • Patients with Renal Impairment (creatinine clearance < 30 ml/min): Longer dosing intervals and a reduced total daily dose may be warranted.

Overdose Effects

  • Liver damage: Possible with 10 gm or more in adults, or 5 gm or more if risk factors are present (e.g., long-term use of liver enzyme-inducing drugs like Carbamazepine, Phenobarbitone, Phenytoin, Primidone, Rifampicin, St. John's Wort; regular excessive alcohol consumption; glutathione depletion due to eating disorders, cystic fibrosis, HIV, starvation, cachexia).
  • Symptoms (first 24 hours): Pallor, nausea, vomiting, anorexia, abdominal pain.
  • Delayed Symptoms (12-48 hours): Liver damage becomes apparent. Abnormalities of glucose metabolism and metabolic acidosis may occur.
  • Severe poisoning: Hepatic failure may progress to encephalopathy, hemorrhage, hypoglycemia, cerebral edema, and death. Acute renal failure (acute tubular necrosis) may develop, even without severe liver damage. Cardiac arrhythmias and pancreatitis have been reported.
  • Immediate Treatment is Essential:
    • Consider activated charcoal if overdose within 1 hour.
    • Measure plasma Reset concentration at 4 hours or later (earlier concentrations are unreliable).
    • N-acetylcysteine is the antidote and can be used up to 24 hours after ingestion, with maximum protective effect up to 8 hours post-ingestion (effectiveness declines sharply thereafter).
    • Oral Methionine may be an alternative in remote areas if vomiting is not a problem.
    • Management of serious hepatic dysfunction beyond 24 hours should involve specialists.
