Rinotin is a tablet containing Levocetirizine Dihydrochloride USP 5 mg. It is manufactured by Biopharma Limited in Bangladesh.

Indications:

Rinotin (Levocetirizine) is a highly effective and well-tolerated non-sedating antihistamine. It is the active enantiomer of cetirizine and works by selectively blocking histamine H1 receptors. It is indicated for the treatment of symptoms associated with various allergic conditions, including:

  • Seasonal Allergic Rhinitis: (e.g., hay fever) characterized by sneezing, runny or itchy nose, watery or itchy eyes.
  • Perennial Allergic Rhinitis: (year-round allergies) with similar symptoms to seasonal allergic rhinitis.
  • Chronic Idiopathic Urticaria: (chronic hives) characterized by persistent itchy skin rashes/hives.

Pharmacology:

Levocetirizine is a potent, selective H1-receptor antagonist. It works by blocking the action of histamine, a natural substance produced by the body during an allergic reaction. By inhibiting histamine, levocetirizine reduces symptoms like itching, swelling, redness, and mucus production associated with allergic responses. It has a high affinity for H1 receptors and a rapid onset of action, providing relief within an hour for many individuals.

Dosage and Administration:

Rinotin tablets are taken orally and can be administered with or without food.

  • Adults and Children over 6 years of age: One Rinotin tablet (5 mg) once daily, preferably in the evening.

Dosage adjustment in renal impairment: Levocetirizine is primarily excreted by the kidneys. Dosage adjustments are necessary for patients with impaired kidney function:

  • Creatinine Clearance (CrCl) 50-80 mL/min: 2.5 mg once daily.
  • CrCl 30-50 mL/min: 2.5 mg once every other day.
  • CrCl 10-30 mL/min: 2.5 mg twice a week.
  • CrCl <10 mL/min or End-Stage Renal Disease/Hemodialysis: Contraindicated.

Side Effects:

Levocetirizine is generally well-tolerated. Common side effects are usually mild and include:

  • Headache
  • Dry mouth
  • Fatigue/Drowsiness (though generally considered "non-sedating," some individuals may experience mild drowsiness, especially at higher doses or with alcohol).
  • Nasopharyngitis (inflammation of the nose and throat)
  • Pharyngitis (sore throat)

Less common or rare side effects can include:

  • Dizziness
  • Abdominal pain, nausea, diarrhea
  • Rash or itching
  • Urinary retention (rare, use with caution in patients with increased risk like prostatic hyperplasia or spinal cord lesions).
  • Rebound pruritus (itching) has been reported after stopping long-term cetirizine use, which may also occur with levocetirizine.

Contraindications:

Rinotin is contraindicated in patients with:

  • Known hypersensitivity to levocetirizine, cetirizine, or any component of the formulation. 
  • End-stage renal disease (CrCl <10 mL/minute) or undergoing hemodialysis.
  • Infants and children 6 months to 11 years of age with renal impairment.

Precautions and Warnings:

  • CNS Depression: May cause CNS depression, affecting mental or physical abilities. Patients should be cautious when performing tasks requiring alertness (e.g., driving, operating machinery), especially with alcohol or other CNS depressants.
  • Renal Impairment: Use with caution in adults with mild to severe renal impairment; dosage adjustments are crucial.
  • Urinary Retention: Use with caution in patients with predisposing factors for urinary retention. Discontinue if urinary retention occurs.
  • Epilepsy/Risk of Convulsions: Use with caution in epileptic patients or those at risk of convulsions.
  • Allergy Testing: Stop taking Rinotin at least three days before an allergy skin test, as it can affect test results.

Pregnancy and Lactation:

  • Pregnancy (US FDA Pregnancy Category B): The safety of levocetirizine in pregnancy has not been definitively established. It should be used with caution during pregnancy and only if the benefits to the mother outweigh any potential risk to the fetus. Consult a physician.
  • Lactation: Levocetirizine is excreted into breast milk. Caution should be exercised, and a physician should be consulted to weigh the risks and benefits for the nursing mother and infant.

Drug Interactions:

  • CNS Depressants: Concomitant use with alcohol or other CNS depressants (e.g., sedatives, tranquilizers) may increase drowsiness and impair performance.
  • Ritonavir: May increase the plasma concentration of levocetirizine, though typically not requiring dosage adjustment.
  • Theophylline: Can cause a small reduction in levocetirizine clearance.
  • Drugs affecting kidney function: Concomitant use with drugs that affect kidney function should be monitored, as levocetirizine is primarily renally excreted.

Storage Conditions:

Store below 30°C, away from light and moisture. Keep out of reach of children.
