Rivamer 4.5 Capsule contains Rivastigmine Tartrate equivalent to Rivastigmine 4.5 mg. It is used to treat mild to moderately severe dementia associated with Alzheimer's disease and dementia in patients with Parkinson's disease.

Indications:

• Symptomatic treatment of mild to moderately severe Alzheimer's dementia. 
• Symptomatic treatment of mild to moderately severe dementia in patients with Idiopathic Parkinson's disease.  

Pharmacology: 

Rivastigmine is a carbamate derivative thatinhibits cholinesterase (acetylcholinesterase and butyrylcholinesterase) in the brain.By inhibiting these enzymes, rivastigmine prevents the breakdown of acetylcholine, a chemical messenger important for memory and thinking.This leads to an increased concentration of acetylcholine in the brain, which can help improve cognitive function in people with dementia. 

Dosage and Administration:

• The usual starting dose is 1.5 mg twice a day.
• The dose may be increased every 2 weeks based on tolerability, to 3 mg twice a day, then 4.5 mg twice a day, and finally to a maximum of 6 mg twice a day.  
• Rivamer capsules should be taken with food, in the morning and evening.

Side Effects:

Common side effects include:

• Nausea  
• Vomiting  
• Loss of appetite  
• Diarrhea  
• Dizziness  
• Headache  
• Weakness  

Less common but potentially serious side effects can occur. It's important to report any unusual symptoms to your doctor.

Precautions:

• Use with caution in patients with a history of peptic ulcers, seizures, asthma or other respiratory problems, heart conditions (including slow heart rate), or liver or kidney disease.  
• Rivastigmine may cause dizziness and drowsiness, so caution should be exercised while driving or operating machinery.  
• Monitor body weight regularly as Rivastigmine can cause weight loss.  

Contraindications:

Rivamer is contraindicated in patients with:

• Hypersensitivity to rivastigmine or other carbamate derivatives.  

Drug Interactions:

Rivastigmine may interact with other medications, including:

• Cholinomimetic substances (drugs that mimic the action of acetylcholine).  
• Anticholinergic medicinal products (drugs that block the action of acetylcholine).  
• Succinylcholine-type muscle relaxants used during anesthesia.

Pregnancy and Lactation:

Rivamer is generally not recommended for use during pregnancy and breastfeeding due to a lack of safety information.

Storage:
