Salflu 250 Rotacap is an inhalation capsule containing a combination of two active ingredients:  

  • Salmeterol (as Salmeterol Xinafoate BP) 50 mcg
  • Fluticasone Propionate BP 250 mcg

It is manufactured by The ACME Laboratories Ltd. in Bangladesh.  

Indications:

Salflu 250 Rotacap is indicated for the regular treatment of asthma and Chronic Obstructive Pulmonary Disease (COPD). It is particularly suitable for patients who:  

  • Are not adequately controlled with inhaled corticosteroids alone and require a long-acting beta2-agonist.  
  • Are already adequately controlled on both an inhaled corticosteroid and a long-acting beta2-agonist.  

This combination aims to protect against symptoms (Salmeterol) and improve lung function while preventing exacerbations (Fluticasone Propionate).It is not for the relief of acute asthma symptoms for which a fast and short-acting bronchodilator (e.g., salbutamol) is required.  

Pharmacology:

  • Salmeterol: A selective, long-acting (12-hour) beta2-adrenoceptor agonist. It works by relaxing the muscles in the airways, leading to bronchodilation and making breathing easier. Its long action provides sustained relief.  
  • Fluticasone Propionate: A corticosteroid with potent anti-inflammatory action specifically within the lungs. It reduces inflammation and swelling in the airways, which helps to control asthma symptoms and prevent exacerbations. At recommended inhaled doses, it has minimal systemic side effects.  

Dosage and Administration:

Salflu 250 Rotacap is designed for inhalation only using a Rotahaler device. The capsule should not be swallowed. The recommended dosage for adults and adolescents (12 years and older) is one Salflu 250 Rotacap twice daily (morning and evening, approximately 12 hours apart).  

  • Important:
    • Patients should be instructed on the proper use of the Rotahaler device to ensure the medication reaches the lungs effectively.  
    • The capsule should be inserted into the Rotahaler immediately before use.  
    • It is advisable to rinse the mouth with water after each inhalation to reduce the risk of oral fungal infections (thrush) and hoarseness.  
    • This medication must be used daily for optimum benefit, even when asymptomatic.  

Children (4 years and older): Salflu 100 Rotacap (Salmeterol 50 mcg + Fluticasone 100 mcg) is typically used for this age group, one capsule twice daily. Salflu 250 is generally not recommended for children under 12 years unless specifically prescribed by a specialist.  

Side Effects:

As Salflu contains both Salmeterol and Fluticasone Propionate, the type and severity of adverse reactions associated with each compound may be expected. There is no increased incidence of additional adverse events following concurrent administration.  

Common side effects (due to Salmeterol):

  • Headache  
  • Tremor, subjective palpitations (these tend to be transient and reduce with regular therapy)  
  • Tachycardia (fast heart rate, in some patients)  
  • Oropharyngeal irritation  

Common side effects (due to Fluticasone Propionate):

  • Candidiasis of the mouth and throat (thrush)  
  • Hoarseness or throat irritation  

Other reported side effects (rare):

  • Hypersensitivity reactions (rash, angioedema)  
  • Muscle cramps, non-specific chest pain, arthralgia  
  • Nausea, dizziness, insomnia  
  • Potentially serious hypokalemia (low potassium levels) can result from beta2-agonist therapy.  
  • Paradoxical bronchospasm (sudden worsening of breathing after inhalation) – if this occurs, discontinue immediately and use a fast-acting bronchodilator.  

Contraindications:

Salflu 250 Rotacap is contraindicated in patients with:  

  • A history of hypersensitivity to Salmeterol, Fluticasone Propionate, or any other component of the formulation.  
  • It should not be initiated in patients during an acute exacerbation of asthma or if they have acutely deteriorating asthma.  

Precautions and Warnings:

  • Not for Acute Relief: Salflu 250 Rotacap is a maintenance therapy and should not be used to treat acute asthma symptoms or severe, rapidly deteriorating asthma attacks. A short-acting bronchodilator should always be available for rescue.  
  • Transfer from Oral Steroids: Special care is needed when transferring patients from systemic oral corticosteroids to inhaled fluticasone propionate due to potential adrenal insufficiency.  
  • Tuberculosis: Administer with caution in patients with active or quiescent pulmonary tuberculosis.  
  • Cardiovascular Disorders: Use with caution in patients with severe cardiovascular disorders, including heart rhythm abnormalities.  
  • Thyrotoxicosis, Diabetes Mellitus, Hypokalemia: Use with caution in patients with these conditions.  
  • Growth Retardation: Orally inhaled corticosteroids may cause a reduction in growth velocity in pediatric patients, though the long-term effects on final adult height are unknown.  
  • Systemic Steroid Effects: Although systemic effects are less likely with inhaled fluticasone, they can occur at high doses or with prolonged use.  
  • Rinsing Mouth: Instruct patients to rinse their mouth with water after each inhalation to minimize the risk of oral candidiasis.  

Pregnancy and Lactation:

  • Pregnancy (US FDA Pregnancy Category C): Use during pregnancy should only be considered if the expected benefit to the mother outweighs any possible risk to the fetus.  
  • Lactation: It is not known whether Fluticasone Propionate is excreted in human breast milk. Salmeterol levels in breast milk are negligible after inhaled therapeutic doses. Caution should be exercised, and a physician should be consulted.  

Drug Interactions:

  • Beta-blockers: Both non-selective and selective beta-blockers should generally be avoided in patients with asthma, as they can block the bronchodilator effect of Salmeterol.  
  • Potent CYP3A4 inhibitors (e.g., Ritonavir, Ketoconazole, Itraconazole): Concomitant use can significantly increase systemic exposure to fluticasone propionate, potentially leading to systemic corticosteroid effects. Co-administration should be avoided unless the benefits outweigh the increased risk of systemic corticosteroid side effects.  
  • Other Sympathomimetics: Use with caution with other sympathomimetic agents due to potential additive effects.  
  • Non-potassium-sparing diuretics: Concurrent use may increase the risk of hypokalemia.  

Storage Conditions:

Store below 30°C, protected from light and moisture. Keep out of the reach of children. The capsules should only be inserted into the Rotahaler device immediately prior to use.
